Bakgrund: Allt fler patienter med obotlig cancer väljer att vårdas i hemmet. I distriktssköterskans arbete ingår det att möta patienters existentiella frågor.  Studier visar att distriktssköterskor upplever att det kan vara svårt och känslosamt att ha existentiella samtal med patienter.

Syfte: Syftet var att belysa distriktssköterskans erfarenhet av att vårda patienter med obotlig cancer med fokus på samtal kring existentiella frågor.

Metod: Semistrukturerade intervjuer valdes för genomförande av studien. Åtta distriktssköterskor intervjuades utifrån en intervjuguide. För att analysera insamlad data användes kvalitativ manifest innehållsanalys.

Resultat: Efter analysprocessen framkom tre kategorier och 11 underkategorier. I kategorin Distriktssköterskans förhållningssätt framkom det många centrala begrepps som ansågs vara av stor vikt vid existentiella samtal för att kunna inta ett professionellt förhållningssätt. Kategorin Faktorer som påverkar samtalet visade att patientens ålder, kön och kultur har stor betydelse för samtalet. Det framkom också att några distriktssköterskor hade erfarenhet av att inte alla patienter med obotlig cancer erbjuds samtal kring existentiella frågor. Kategorin Innehåll i samtalet beskrev samtal kring meningen med livet och döden, samtal kring relationer, hopp, ångest och oro.

Slutsats: Några av distriktssköterskorna hade erfarenhet av att patienter med obotlig cancer inte alltid erbjuds existentiella samtal. Distriktssköterskorna upplever ibland dessa samtal som smärtsamma och svåra och anser att det behövs mer utbildning och erfarenhet kring existentiella samtal.

Background: An increasing number of patients with incurable cancer prefer to be cared for at home. One part of the district nurse's duties is to meet questions from patients that are related to existential issues. Studies have shown that district nurses are facing challenges as well as emotional distress when engaging in existential conversations with patients.

Aim: The aim of this study was to highlight district nurses´ experiences of caring for patients with terminal cancer with a focus on existential questions.

Method: A qualitative research method with semi-structured interviews was chosen for this study. We conducted interviews with eight district nurses using an interview guide. The collected data was analyzed with a qualitative manifest content analysis.

Results: As a result of the analysis process, three main categories and eleven subcategories emerged. The category The District Nurse’s Approach reveals many key concepts that are considered to be of great importance to existential conversations in order to support a professional approach. The category Factors that Influence the Conversation illustrates that the patient's age, gender and culture has a major impact on the outcome of the conversation. It was also revealed that existential conversations are not offered to all patients. The category Content of the Conversation describes conversations about the meaning of life and death, as well as conversation about relationships, hope, anxiety and worry.

Conclusion: District nurses´ experience is that not all patients with terminal cancer are offered conversations about existential issues. The district nurses sometimes perceive these conversations painful as well as difficult, and they also request more education and more experience on this field.
