Mikroskopisk bedömning av urinsediments hållbarhet utan och med tillsatt konserveringsmedel med OLYMPUS BX53

Abstract [en]

Urine microscopy can be used for determination of renal function and the analysis is of value in diagnostics of various kidney diseases, as well as follow-up tests, kidney transplantations and intoxications. The components that may occur in urine sediment are among others erytrocytes, leukocytes, epithelial cells and casts. The purpose of this study was to examine the durability of urine sediment without and with added preservatives, to determine how long the samples can stand before they change. In this study, urine samples collected from 29 patients (19 men and 10 females, 30-87 years) with deteriorating kidney function were used. From each patient, 10 mL of urine were collected into 4 tubes. The samples were analyzed with test strips (Urilyzer® 100Pro) for screening of acetoacetate, glucose, protein, erythrocytes, nitrite, leukocytes and pH in urine. The preservative, sodium thiocyanate, was added to 20 patient samples which were evaluated in the microscope OLYMPUS BX53 after 8, 24, 48 and 72 h. Twenty patient samples without added preservatives were assessed at 4, 8, 24 and 48 h. The result showed no destruction of erythrocytes, leukocytes, epithelial cells, granulated cylinders or wax cylinders in neither samples without or with added preservative. A variation showed in the amount of hyaline casts in both samples without and with added preservatives, but the difference wasn’t considered being of vital clinical importance. There was no substantial difference when comparing the sustainability of urine samples without and with added preservatives. Urine sediment without added preservatives were considered durable in 24 h when stored in fridge (2-8 °C) and urine sediment with added preservatives in 48 h when stored in fridge (2-8 °C).

Urinsediment kan användas för bestämning av njurfunktionen och analysen är av värde vid diagnostik och uppföljning av olika njursjukdomar, bland annat akut njursvikt och glomerulonefrit, samt vid njurtransplantationer och intoxikationer. Komponenterna som kan förekomma i urinsediment är bland annat erytrocyter, leukocyter, epitelceller och cylindrar. Syftet med studien var att undersöka hållbarheten på urinsediment utan och med tillsatt konserveringsmedel för att avgöra hur länge sedimenten kan stå innan de förändras. I denna studie användes 29 patienter (19 män och 10 kvinnor mellan 30-87 år) med nedsatt njurfunktion. Från varje patient samlades 4 rör innehållande 10 mL urin och proverna analyserades med testremsa (Urilyzer® 100Pro) för screening av acetoacetat, glukos, protein, erytrocyter, nitrit, leukocyter och pH i urin. Till 20 patientprover tillsattes konserveringsmedlet natriumtiocyanat och proverna bedömdes i mikroskopet OLYMPUS BX53 vid 8, 24, 48 och 72 tim efter provtagning. Tjugo patientprover utan tillsatt konserveringmedel bedömdes efter 4, 8, 24 och 48 tim. Resultatet visade att det inte var någon skillnad i antal erytrocyter, leukocyter, epitelceller, granulerade cylindrar eller vaxcylindrar i varken prover utan eller med tillsatt konserveringsmedel. Det fanns en variation i mängden hyalina cylindrar över tid i både prover utan och med tillsatt konserveringsmedel, men skillnaden ansågs inte ha avgörande klinisk betydelse. Det var ingen avsevärd skillnad vid jämförelsen i hållbarhet mellan urinprover utan och med tillsatt konserveringsmedel. Urinsediment utan tillsatt konserveringsmedel anses vara hållbara i 24 tim vid förvaring i kyla (2-8 °C) och urinsediment med tillsatt konserveringsmedel i 48 tim i kyla (2-8 °C).
