    Abstract [en]

    The mean scores of the Creative Climate Questionnaire for 152 staff members working in 58 sheltered homes for individuals with autism wererated closer to creative than to stagnant organizations. There was a significant difference between two types of settings. In sheltered homes, thepersons with autism had their daily activity outside the home. Here staff members had higher scores in all creative dimensions compared tohomes with daily activities. Staff in sheltered homes, where the residents leave the premises for their daily work or activities, perceived the workclimate as significantly more creative and satisfying.

    Parents experienced their child with autism´s abberant behaviours not possible to handle also social isolation, negative effects on the siblings that made them leave the child with autism to a group home. Two years later the experienced relief for the family, due to the group home arrengement and the childs improvement, but with an ethical dilemma which made them feel guilty. despite increased hope for the future.

    Abstract [en]

    The aim of this study was to describe the present and past experiences of 14 siblings from five families in terms of having abrother or sister with autism and mental retardation. Personal interviews were conducted with the siblings before their brothersor sisters were moved to a newly opened group home. Qualitative content analysis was used for the analysis of the transcribedtexts. The analysis resulted in seven content categories: precocious responsibility, feeling sorry, exposed to frightening behavior,empathetic feelings, hoping that a group home will be a relief, physical violence made siblings feel unsafe and anxious, andrelations with friends were affected negatively. The conclusion is that these siblings' experiences revealed stressful lifeconditions. Counseling for the family and for siblings is recommended to help them deal with their feelings and problems. Forthe siblings in these five families, a group home was a relevant alternative as a temporary or permanent placement for the childwith autism and mental retardation.

    Implementation of user organizations in Swedish health care and social services for persons with substance use disorders2019In: Nordic Social Work Research, ISSN 2156-857X, E-ISSN 2156-8588Article in journal (Refereed)
    Abstract [en]

    Background: In Sweden, the National Guidelines for Substance Abuse Treatment includes new recommendations concerning integrating a user perspective. From 2009 to 2014, the Swedish National Board of Health and Welfare supported and financed the development of organizations serving regional users. The programme was evaluated, and the results showed a development of user influence in substance use treatment services in 20 out of 21 regions by 2014. The implementation of user organizations’ influence was evaluated in 2018 for this follow-up study.

    Aim: The aim of this four-year follow-up study was to evaluate whether user organizations continued existing once national support ended, and whether their influence in social services and health care, at both organizational and individual levels had been implemented.

    Method: A questionnaire was dispatched to 20 regional user organizations. It included information and questions focused on their situation, and on participants’ experiences of the influence of user organizations in health care and social service.

    Results: The results obtained from the questionnaire show that 14 out of 20 user organizations still existed and reported a developed influence in social services and health care at organizational and individual levels. National support was perceived as necessary for the ongoing development of user organizations and to continue increasing their influence.

    Conclusions: Support at the national level initiated the development of user influence in Sweden, which has increased at both organizational and individual levels in the context of social services, as well as in health care for persons with substance use disorders.

    Abstract [en]

    This article describes the results from a cooperative model project treating delinquents and their families with Functional Family Therapy (FFT). The treatment has taken place within a frame of cooperation between social welfare, child psychiatry and a drugtreatment unit. The results from the treatment group are compaired with treatment as usual. The FFT-group showed increased familyfunction and fewer psychiatric symtoms after treatment. Both patents and youths showed higer optimism and valued the treatment high. The results are smilar to earler university based research with the same metod.

    Abstract [sv]

    ADHD En funktionsnedsättning med debut i baranåren. Kärmsymtom karaktäriseras av uppmärksamhetsproblem, impulsivitet ioch hyperaktivitet.

    I ett antal fall sker en normalisering eller mognadsprocess, i andra fall kan någon form av psykisk ohälsa förekommma samtidigt. Den diagnostiska utredningen är omfattande, och både instrument för diagnostik och den diagnostiska processen bör undersökas bättre.

    Många olika insatser och behandlingar, förutom läkemedel förekommer idag, men kunskapen om eras nytta, risker och kostnader måste förbättras. Vissa läkemedel lindrar ADHD symtom vid korttidsbehandling, men nyttan av långtidsbehandling går inte att bedöma. Vanliga biverjkningar av dessa läkemedel är illamående och nedsatt aptit, för barn viktminskning och pulsökning.

    Abstract [en]

    Evaluation of managers, primary care physicians and health coordinators work adapted to the National guidelines for substance abuse and addiction treatment was carried out 2012 in health centers in KalmarCounty in Sweden. The evaluation was funded by the Regional Council in KalmarCounty and implemented by LinnaeusUniversity. The evaluation aims to describe operations managers, primary care physichians and health coordinators principles and norms, using the National Guidelines for abuse and addiction treatment.

    Recommendations in the National guidelines to the swedish primary health care is to meet patients with risk use of alcohol and drugs with motivational interviewing. The aim is to support patients to change their habits towards a healthier lifestyle.

    Data collection consisted of questionnaires to operations managers and interviews with doctors and health coordinators, ie nurses with specialized training in the field. Questions were designed based on the guidelines witch are advisory in the field: the National Guidelines for substance abuse and addiction care (National Board of Health, 2007) and the regional guidelines (2010) for primary care, health centers in Kalmar County. The guidelines focus on the identification, counseling and treatment to patients at risk and abuse of alcohol and drugs.

    The results presented will include:

    1. Operations managers at KalmarCounty health centers experience that they have not received training in the National Guidelines to the extent they would need.
    2. Operations managers found that patients with risk and substance abuse to a greater extent are social service responsibilities, but partly also the primary care      responsibilities.
    3. Doctors and health coordinators mean that it is the responsibility of health care to detect patients with risk drinking and to help patients with hazardous drinking or drug use to find the best place for treatment and care.
    4. Few managers, doctors and health coordinators knew the guidelines' recommendations for health center patients. Despite this, several health centers use procedures for the identification and treatment based on the guidelines recommendations. Some health centers offered staff training in MI, Motivational interview and used screening instruments, including alcohol consumption questions.
    5. Only health coordinators routinely used screening instruments, they were the only profession trained in this.
    6. Doctors experienced that they did not have time enough and also that it would interfere with the patients´ integrity to question them about alcohol consumption.
    7. Biological markers and screening instruments were used frequently in most health centers, and more than 50% of them were offering Motivational interviews, Relapse prevention and Cognitive behavioral therapy to the patients.

    Proposal for improvement area based on the results:

    1. Continued education about early detection of hazardous according to the National Guidelines for managers, doctors and health coordinators.
    2. Mandate issue for busines managers should be discussed, because prerequisites for implementation are dependent on the manager's mandate.
    3. Tutorials, method support including training to question patients about their alcohol consumption may be implemented at network meetings.
    4. Method Support / training in the use of screening instruments, lifestyle questionnaires and surveys that can facilitate on issues perceived to be taboo, such as the patient's alcohol consumption.
    5. A policy that clarifies responsibilities, which should do what when it comes to alcohol issues.
    6. Increase collaboration and delegation between health coordinators and doctors but also to another profession on some health centers, alcohol therapists.
    7. Education / training / house training on the relationship between alcohol and somatic consequences.
