    Organizational climate and related factors are associated with outcome and are as such of vital interest for healthcare organizations. Organizational Readiness for Change (ORC) is the questionnaire used in the present study to assess the influence of organizational factors on implementation success. The respondents were employed in one of 203 Swedish municipalities within social work and psychiatric substance/abuse treatment services. They took part in a nationwide implementation project organized by the Swedish Association of Local Authorities and Regions (SALAR), commissioned by the Swedish National Board of Health and Welfare. Aim: The aims were: (a) to identify classes (dusters) of employees with different ORC profiles on the basis of data collected in 2011 and (b) to investigate ORC profiles which predicted the use of assessment instruments, therapy methods and collaborative activities in 2011 and 2013. Design and recruitment: The evaluation study applied a naturalistic design with registration of outcome at consecutive assessments. The participants were contacted via official e-mail addresses in their respective healthcare units and were encouraged by their officials to participate on a voluntary basis. Statistics: Descriptive statistics were obtained using SPSS version 23. A latent profile analysis (LPA) using Mplus 73 was performed with a robust maximum likelihood estimator (MLR) to identify subgroups (clusters) based on the 18 ORC indexes. Results: A total of 2402 employees responded to the survey, of whom 1794 (74.7%) completed the ORC scores. Descriptive analysis indicated that the respondents were a homogenous group of employees, where women (72.0%) formed the majority. Cronbach's alpha for the 18 ORC indexes ranged from alpha = 0.67 to alpha = 0.78. A principal component analysis yielded a four -factor solution explaining 62% of the variance in total ORC scores. The factors were: motivational readiness (alpha = 0.64), institutional resources (alpha = 0.52), staff attributes (alpha = 0.76), and organizational climate (alpha = 0.74). An LPA analysis of the four factors with their three distinct profiles provided the best data fit: Profile 3 (n = 614), Profile 2 (n = 934), and Profile 1 (n = 246). Respondents with the most favorable ORC scores (Profile 3) used significantly more instruments and more treatment methods and had a better collaborating network in 2011 as well as in 2013 compared to members in Profile 1, the least successful profile. Conclusion: In a large sample of social work and healthcare professionals, ORC scores reflecting higher institutional resources, staff attributes and organizational climate and lower motivational readiness for change were associated with a successful implementation of good practice guidelines for the care and treatment of substance users in Sweden. Low motivational readiness as a construct may indicate satisfaction with the present situation. As ORC proved to be an indicator of successful dissemination of evidence-based guidelines into routine and specialist healthcare, it can be used to tailor interventions to individual employees or services and to improve the dissemination of and compliance with guidelines for the treatment of substance users.     Abstract [en]

    Studied the relationship between motivation and approaches to moral decision-making, and the emotions experienced in moral dilemmas. 44 students were interviewed about a dilemma they had faced. Intimacy was related to a preference for making decisions after having consulted others and to being open to their values and norms, whereas achievement was related to consequence-oriented reasoning and concrete construal of moral problems. When asked to relive their moral dilemma, participants scored lower than their baseline level on a mood questionnaire, females scoring lower than the males. Possible implications of emotion on cognitive processes involved in solving moral dilemmas are discussed.

    Abstract [en]

    The chapter by definition focuses on Swedish research and treatment practice in the area of comorbiditity. Three “ethiological” lines of comorbidity is outlined. it thus seems warranted to look upon patients with a precipitating psychiatric diagnosis and a consequent problem of alcohol and/or substance abuse as one specific model/type (1). A second type representting the drug and substance dependent person who´s psychiatric and psychological problems often remain unattended until a more thorough assessment is made. Here, personality disorders are by far the most common disorder. Model 3, finally, corresponds to a number of alcohol- and substance dependent persons who are not afflicted by any particular chronical psychiatric or psychological problem despite the severity of the alcohol/substance problem. The reason for differentiating specific comorbid patterns are, apart from diagnostic utility, also the need to gauge treatment carefully. The immediate ways of approaching the patient in a treatment encounter as well as for choosing an adequate treatment intervention differ and contain different possible combinations of psychosocial and pharmacological interventions. The national records of the Swedish National Board of Health and Welfare underreport systematically due to the fact the diagnostic procedures is lacking in clinical treatment and diagnoses consequently reported scarcely. The situations within the hospital system, in prisons and other treatment settings are presented, leaving a clear picture of a heavily afflicted group of patients, where their psychiatric ailments are only dealt with in a few clinics. In drug dependent individuals (type 2), personality disorders are be far the most common problem ranging from 60–85% in clinical settings.

    Abstract [en]

    All randomized controlled studies published in international peer-reviewed journals up to July 1999 were meta-analysed with the aim of studying the effect of psychosocial interventions on patients with drug dependence. Systematic literature searches were administered by SBU. In all 104 studies were screened and rated according to the 12 dimension quality score of SBU. The analyses in the chapter studied the effects on drug abuse and retention and aimed at specific domains of research; 1) efficacy of treatment of opiate abuse/ depen¬dence (n=24) containing 5.721 patients, 2) of cocaine abuse/dependence, 26 studies with 5.956 patients), 3) cannabis abuse, (7 studies with 707 patients), 4) of treatment methods for patients with mental illness and (9 studies), 5) residential treatment containing 9 studies and 4.867 patients, 6) finally for structure interventions with other outcome measures than drug use, 5 studies with 461 patients. A clinical methodological model elaborated from Wolberg´s (1988) classification, divided the interventions into three groups: ranging in therapist skills, goals and costs from a) supportive, b) reeducational to c) psychotherapeutic intervenetions. Effect sizes were calcylated using Hedge´s d and groups of interventions were weighted with the root of the variance. In opiate abuse re-educational and psycho-therapeutic interventions had a significant but low Wd of .23 and .29 respectively. Re-educational interventions only gained a significant but low effect size of Wd = .24 in treatment of cocaine abuse. Family therapies finally were efficient in the treatment of teenage cannabis abusers in with an intact family network. Supportive interventions, generally, were not effective. For retention as outcome measure, finally, psychotherapies had a significant medium effect size in retention of opiate abuse (Wd= .56, n=3.380) and a low but significant positive effect size in retention of cocaine abuse (Wd= .30, n=3.381).

    En representativ sample drogberoende patienter på sjukhus 1988 - 89 (n=125) följdes upp vid 5 år och vid 15 år. Kliniska ata och utfallsdata registrerades vid indevårdtillfället och vid uppföljningarna med en semistrukturerad intervju omfattande demografiska karakteristika, kliniska diagnoser kompletterade med kliniska test och skattningsskalor. Registerdata används för validering och jämförelse. Säkre utfallsdata vid 5 år finns registrerade för 112 personer av hela samplet (90 %) och för 110 (88 %) vid 15 år.

    Abstract [en]

    Aim: To assess the diagnostic concordance of SCID-II and clinicians' estimation of DSM-III-R personality disorders of substance abusers. Method: Clinical diagnoses of substance abusers in inpatient treatment were compared with SCID-II diagnoses (N = 138). Findings: The overall prevalence of personality disorder was 79% for clinical diagnosis and 80% for SCID-H diagnosis. Substantial agreement was found for borderline personality disorder, and moderate agreement was found for presence of any personality disorder, and antisocial personality disorder. All other disorders had slight to fair agreement. Antisocial personality disorder was overdiagnosed by clinical diagnosis but schizotypal, obsessive-compulsive, passive-aggressive, and masochistic personality disorders were reported more often by SCID-II. Selecting only the primary clinical diagnosis and omitting additional clinical diagnoses, reduced agreement with SCID-II diagnoses. Implications: Clinical diagnosis and structured interviews are not interchangeable, and produce somewhat different profiles of diagnoses for a group of substance abusers, but the two methods for diagnosing personality disorders converge for the two most common personality disorders in substance abusers. Rare and less-known diagnoses tend to be underreported whereas common and well-known disorders tend to be slightly overdiagnosed by clinical diagnosis as compared with a semistructured interview, especially if only one clinical diagnosis is noted.

    Abstract [en]

    Previous research has shown that most transitions into abstinence happens in the stages of the drug career quickly after the first treatment episode. Mortality is somewhat reduced for patients who become abstinent, but remains high for patients who remain addicted. However, even among substance abusers who become abstinent, mortality is often higher than in the general population. A consecutive sample of drug users admitted for detoxification was followed for 15 years. Face-to-face interviews were conducted at 5-year follow-up. At 15-year follow-up, 24% were dead. Cox proportional hazard regression was conducted to predict mortality for continuous variables, and Gehan's Wilcoxon test was used to predict mortality for dichotomous variables. Psychiatric status at 5-year follow-up was predictive of 15-year mortality, whereas abstinence was not. Subjects who later died had higher scores on the Symptom Checklist 90 [SCL-90] Global Severity Index, lower meaningfulness on the Sense of Coherence scale, and lower Global Assessment of Functioning [GAF] scores at 5-year follow-up. By contrast, there were no associations between baseline drug use and antisocial personality disorder diagnoses and mortality.
