Färguppfattning genom Multilens färgade filter2019Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

Aim: The aim of this study was to see if there would be any difference in color vision perception between two different Multilens prototype filter-lenses product for people with color vision deficiencies in the red-green spectrum. The filters were also tested in normal individuals who acted like as controls.

Methods: 32 people participated in this study. 13 had a color vision deficiency in the red-green spectrum and 19 of them had normal color vision. Two of Multilens new filter-lens prototypes, filter B and filter D, was tested against no filter. Instrument used was the Cambridge Research System´s Metropsis. The test for measurements was CCT-Ellipse 10a, which presented results in terms of ellipses. The necessary values were analysed by Friedman test.

Results: Multilens filter-lenses gave a significant change in color perception for both subjects with normal- or deficient color vision. A significant change in color vision between filter B and filter D was shown for those with normal color vision. Subjects with deficient color vision only experienced a significant change with filter D. The subjective impression of the filter-lenses was generally positive for those with deficient color vision.

Conclusion: Filter B and D gives an improvement on color vision for some of the participants but is not statistically significant. All participants experienced changed color vision, but when a specific hue became easier to see, other hues became more difficult to see. Subjectively, some of the participants with color deficiency saw a big positive difference between with and without the filter-lenses.

The use of informal care by people with vision impairment2018In: PLoS ONE, ISSN 1932-6203, E-ISSN 1932-6203, Vol. 13, no 6, article id e0198631Article in journal (Refereed)
    Abstract [en]

    Purpose

To estimate and characterize the use of informal care by people with vision impairment in Portugal.

    Methods

A total of 546 visually impaired individuals were recruited from Portuguese hospitals. Clinical information was obtained from medical records, socio-demographic details and informal care use were collected during face-to-face interviews. In addition, participants responded to a functional vision questionnaire (activity inventory) to assess their visual ability. Logistic regression was used to determine independent factors associated with informal care use and linear regression was used to determine independent predictors of intensity of informal care use.

    Results

Informal care was reported by 39.6% of the participants. The probability of reporting informal care was higher in non-married, those with comorbidities, with lower visual ability and worse visual acuity. The median number of caregivers’ hours per year was 390 (mean = 470; 95%CI = 488–407), which represent a median opportunity cost of €2,586. Visual ability was the only independent predictor of number of hours of informal care received.

    Conclusions

Informal care was frequently used by individuals with impaired vision. Improving visual ability of people with impaired vision when performing valued activities may reduce the burden of visual loss at personal and societal level. This could be achieved with person-centred visual rehabilitation.

Sjuksköterskors upplevelser av arbetsrelaterad stress i sjukhusmiljö: En kvalitativ intervjustudie2017Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

    Abstract [sv]

Patienters upplevelser av sluten psykiatrisk tvångsvård.: En litteraturstudie.2012Independent thesis Advanced level (degree of Master (One Year)), 40 credits / 60 HE creditsStudent thesis
    Abstract [sv]

Domestic violence: a cross-sectional study among pregnant women in different regions of Sri Lanka2018In: BMJ Open, ISSN 2044-6055, E-ISSN 2044-6055, Vol. 8, no 2, article id e017745Article in journal (Refereed)
    Abstract [en]

Objectives The aims of this study were to assess the regional differences in domestic violence among pregnant women in the capital district and in the tea plantation sector of Sri Lanka, to explore potential contributory factors and to assess whether healthcare workers addressed domestic violence and disclosure among survivors. Design A cross-sectional study was carried out using interviewer-administered Abuse Assessment Screen. Setting Fifty-seven antenatal clinic centres in the capital district and 30 in the tea plantation sector. Participants Pregnant women between 6 and 40 weeks of gestational age. In the capital district, 1375 women were recruited from antenatal clinic centres in the urban (n= 25) and in the rural areas (n= 32), and 800 women from 30 centres in the tea plantation sector. The response rate in the capital district was 95.6% and 96.7% in the tea plantation sector. Results Among the total sample of pregnant women (n= 2088), the prevalence of 'ever abused' was 38.6%, and the prevalence of 'currently abused' was 15.9%. 'Ever abused' (31.5% vs 50.8%) and 'currently abused' (10% vs 25.8%) were significantly higher (P< 0.001) among the women living in the tea plantation sector. 'Ever abused' was associated with living in the tea plantation sector, being employed, living far from gender-based violence care centre and of Muslim ethnicity, after adjusting for age, education and family income. Only 38.8% of all participants had been asked by healthcare workers about abuse. Living in the tea plantation sector and lower level of education were associated with not being asked. Among those who reported 'ever abused', only 8.7% had disclosed the experience to a healthcare worker. Conclusion Domestic violence was prevalent and highest among women in the tea plantation sector compared with the capital district. The capacity of healthcare workers in addressing domestic violence should be increased.

What is the reward?: Medical students’ learning and personal development during a research project course2015In: Medical Education Online, ISSN 1087-2981, E-ISSN 1087-2981, Vol. 20, p. 1-9, article id 28441Article in journal (Refereed)
    Abstract [en]

Background: Until recently, the outcome of medical students’ research projects has mainly been assessed in terms of scientific publications, whereas other results important for students’ development have been less studied. The aim of this study was to investigate medical students’ experiences of learning as an outcome of the research project course.

Method: Written reflections of 50 students were analyzed by manifest inductive content analysis.

Results: Three categories emerged: ‘thinking as a scientist’, ‘working as a scientist’, and ‘personal development’. Students became more aware about the nature of knowledge, how to generate new knowledge, and developed skills in scientific thinking and critical appraisal. Unexpectedly, effects on personal characteristics, such as self-confidence, self-discipline, independence, and time management skills were also acknowledged.

Conclusions: We conclude that individual research projects enhance research-specific skills and competencies needed in evidence-based clinical work and are beneficial for personal and professional development.

Family sense of coherence and its associations with hope, anxiety and symptoms of depression in persons with cancer in palliative phase and their family members: A cross-sectional study2019In: Palliative Medicine: A Multiprofessional Journal, ISSN 0269-2163, E-ISSN 1477-030X, Vol. 33, no 10, p. 1310-1318Article in journal (Refereed)
    Abstract [en]

Background: There is evidence indicating that family sense of coherence predicts quality of family life and promotes family well-being. In families living with the palliative phase of cancer, low hope, anxiety and symptoms of depression are common in both persons with cancer and their family members. Aim: To determine whether family sense of coherence was associated with hope, anxiety and symptoms of depression, respectively, in persons with cancer in the palliative phase and their family members. Design: An observational, cross-sectional, multicentre study was conducted. Nested linear regression analyses were performed in two blocks to determine whether family sense of coherence was associated with hope, anxiety and symptoms of depression. Setting/participants: Persons with cancer (n = 179) and their family members (n = 165) were recruited from two oncology clinics and two palliative centres in three regions in Sweden. Results: The main findings showed that family sense of coherence was significantly and independently associated with hope, anxiety and symptoms of depression. Stronger family sense of coherence was associated with higher hope and lower anxiety and symptoms of depression levels in both persons with cancer and their family members. Conclusion: Health care providers should strive to identify families with weak family sense of coherence, because of its associations with hope, anxiety and symptoms of depression, in order to offer them professional support and thereby achieve increased well-being during the palliative phase of cancer. Future studies should expand our knowledge of family sense of coherence and how to identify families at risk of lower levels of well-being.

Patient participation for safer health care – interviews with physicians and nurses

Patient participation to achieve safer care is an area of growing policy, research and health care management and practice interest. Patients are uniquely placed to observe their treatment, care and physical environment throughout their journey in the health care system. However, very few studies have investigated health care providers’ attitudes and beliefs concerning patient participation for improved patient safety. This study explored factors that acted as facilitators and/or barriers to patient participation for safer care, as perceived by physicians and nurses in Swedish health care. Interviews were conducted with 13 physicians and 11 nurses, using a purposeful sampling strategy to achieve a heterogeneous sample of providers. We identified nine categories of factors, many of which functioned as barriers to patient participation to achieve safer care.

PD 3.1 to the rescue: challenges for participatory design in a health care context2010In: PDC '10 Proceedings of the 11th Biennial Participatory Design Conference, 2010, p. 269-270Conference paper (Refereed)
    Abstract [en]

A National Strategy for E-health has been introduced in Swedish county councils. The strategy indicates that health care needs to become more accessible. To generate usable and sustainable e- Health solutions in Swedish health care, Participatory Design (PD) was introduced as a working method in an e-Health project in the south of Sweden. The project has met with opposition; e-health solutions are not exactly what Swedish health care wanted at the same time as different arenas within the health care organization have difficulties understanding each other. The aim of this study is to find work methods that result in applicable, usable, and sustainable ICT- solutions in every day work within Swedish health care. The study suggests that a modification of third generation of PD may be one way to the challenges PD has come across in the health care context.

Att leva istället för att överleva med cystisk fibros: En kvalitativ intervjustudie som belyser hur livskvalité kan uppnås trots en dödlig sjukdom2019Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

    Abstract [sv]

Pedagogical approaches in quality improvement coaching in healthcare: A Swedish case study of how improvement coaches approach learning in a contemporary healthcare system2015In: Nordic Journal of Studies in Educational Policy, ISSN 2002-0317, Vol. 1, article id 30178Article in journal (Refereed)
    Abstract [en]

In this study we adopt a critical perspective and explore different coaching styles in quality improvement (QI) work in the provision of healthcare. Coaching has gained attention as an effective way to enhance QI in healthcare. This study investigates how coaching is realized in terms of learning: What kinds of learning ideals pervade QI coaching, and how is support for learning realized, given the prevailing conditions in a contemporary healthcare system? For the purpose of this case study, a group of coaches exchanged experiences of their pedagogic roles and the strategies that they employ, on four occasions, over a period of four months. The conversations were filmed and then analyzed, using critical discourse analysis as an analytic framework. Three parallel styles of coaching were identified, which were symbolized by (i) a pointing, (ii) a bypassing, and (iii) a guiding discourse. No persistent dominance of any one of the discourses was found, which suggests that there exists an ever-present tension between a pointing and a guiding pedagogy in the coaching activities. The findings indicate that QI coaching in healthcare is more complex than previous conceptualizations of coaching. Additionally, the findings present a new, ‘bypassing’ coaching style which the coaches themselves were not fully aware of.

Kan skolbaserad fysisk aktivitet påverka elevers skolprestationer?: En litteraturstudie2018Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

Background: In today's society, children and young people spend a lot of their awake time with quiet activities in their free time, such computer use. It contributes to little physical activity and a passive lifestyle. The curriculum for elementary school, preschool class and leisure time (2011, Rev. 2Nf 017) emphasizes the importance of offering all students physical activity during school day. Research shows that physical activity inter alia affects the brain and pupils' school results.

Purpose: The purpose of this literature study was to study what research says about school-based physical activity and its possible impact on pupils (7-15 years) school performance? Method: A systematically studied literature study involving research in the field of school-based physical activity and pupils' school achievements. The literature search was done in the databases One Search, ERIC and PubMed. Inclusions and exclusion criteria were used to demarcate the search. The study selected seven articles that focused on school-based physical activity and included students between the ages of 7- 15 years. The articles were reviewed and evaluated by SBU's review template to see its relevance to the study. The articles are presented in a table of purpose, design and results.

Result: The articles in showed different results. Most results were optimistic and showed that school-based physical activity positively affected pupils' school performance. Other studies show a slight impact or no impact. School-based moderate to vigorous physical activity showed positive improvement in students' academic study results. Aerobic exercise proved to be more effective than moderate physical activity.

Conclusion: Promoting physical activity in schools through implementation of curriculum-based intervention programs can improve pupils' school performance in terms of better academic outcomes and concentration. Especially with the academically weakest students. This indicates that there are positive links between physical activity and pupils' school achievements.

Effekten av glukosmätningsmetoden FGM (LIBRE) på glukoskontrollen mätt som HbA1c i löpande diabetologisk rutinsjukvård2016Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

Background: Type 1 diabetes is an endocrine autoimmune disease that requires daily insulin therapy and careful monitoring of blood glucose. The disease causes a chronic high blood glucose level which is the basis of diabetes complications later in life. Previous studies have shown that intensive treatment of diabetes mellitus has a positive effect by reducing the risk of diabetes complications.

Purpose: The aim of this study was to measure the effect of the glucose measurement method FGM (LIBRE) as measured by HbA1c at current diabetologic routine medical care.

Method: The study is a retrospective medical record study conducted together with endocrine recipiency at Ängelholms hospital.

Results: FreeStyle LIBRE glucose measurement method leads to improvement in HbA1c in diabetes patients with a baseline HbA1c >62,4 mmol/mol. Patients with a baseline HbA1c of 70 mmol/mol reducing their HbA1c with 5 mmol/mol by using FreeStyle LIBRE.

Conclusion: For diabetes patients with a baseline HbA1c >62,4 mmol/mol, use of FreeStyle LIBRE as part of an intensive treatment regime is likely to lead to a reduction in HbA1c. By lowering HbA1c and therby blood glucose levels, the risk for the most common diabetes complications retinopathy, nephropathy, neuropathy and cardiovascular diseases is likely to be reduced.

Using the Internet as source of information during pregnancy: a descriptive cross-sectional study among fathers-to-be in Sweden2018In: Midwifery, ISSN 0266-6138, E-ISSN 1532-3099, Vol. 62, p. 146-150Article in journal (Refereed)
    Abstract [en]

    Objective

The aim of this study was to identify how fathers-to-be used the Internet as a source of information during their partners' pregnancy and how it affected them.

    Design and setting

A descriptive cross-sectional study was conducted. Data were collected through a questionnaire and distributed at a maternity clinic in south of Sweden. The data were analysed descriptively.

    Participants

Ninety-two Swedish fathers participated in the study, and the response rate was 98.9%.

    Findings

Of all the fathers-to-be, 76% sought pregnancy-related information on the Internet. One sought information on a daily basis, 40.6% every week and 58% every month or more rarely. The fathers-to-be who participated at all/most visits at antenatal care searched for information on the Internet more often than those who only attended few/no visits (p = 0.012). A total of 33.4% of fathers-to-be had been recommended a web page by the midwife at the antenatal care. The main reason for using the Internet was to find information about pregnancy related subjects and read about people in similar situations. More than half of the fathers-to-be (61.8%) had at some point been worried by something they read online. These concerns were commonly addressed by asking the midwife at their next appointment (33.9%). Almost 26% of the fathers-to-be chose not to take any action at all to address their concerns.

    Conclusion

The majority of all fathers-to-be searched for information on the Internet, and more than half of the fathers were, at some point, worried about the information they read on the Internet. One way to address questions and concerns could be for the fathers-to-be to ask and discuss with the midwives what they read online so that midwives can recommend appropriate and credible websites. To achieve this, there must be opportunities for midwives to gain knowledge on how best to use the Internet as a tool.

Possible effects on participation in cervical cancer screening and sexual behaviour following HPV vaccination in Sweden2012In: 28th International Papillomavirus Conference & Clinical and Public Health Workshops: abstracts book - epidemiology / public health, 2012, p. 405-405Conference paper (Refereed)
    Abstract [en]

Objective. To illustrate attitudes towards future cervical cancer screening and future sexual behavior among young HPV vaccinated women and parents of HPV vaccinated girls.

    Design. Study with qualitative approach.

Setting. Mid-sized cities in the south of Sweden.

Sample. Young women aged 17 to 26 years (n=17) and parents of girls aged 13 to 17 years (n=6) vaccinated against HPV.

Method. Individual semi-structured interviews were analyzed with qualitative content analysis.

Results. The attitudes towards future participation in cervical cancer screening was positive in both groups, but nearly half of the young women did not know what cervical cancer screening meant. Among the young women, there was a lack of understanding about the relation between sexual transmission, HPV and cervical cancer. The parents had better understanding. Future sexual behavior was thought would not be affected by the vaccination, but participants discussed the possibility of it leading to increased sexual risk taking among others. We could also see that discussions prior to the vaccination increased knowledge about sexually transmitted infections and encouraged conversations about safe sex between parents and daughters.

Conclusions. Young HPV vaccinated women are unaware of cervical cancer screening and to what extent HPV vaccination offers protection. Accurate information about HPV, sexual transmission and cervical cancer is important to prevent possible negative effects on participation in cervical cancer screening and on future sexual behavior.

The effect of women’s leg posture on gazing behavior and perceived attractiveness2018In: Current Psychology, ISSN 1046-1310, E-ISSN 1936-4733, p. 1-6Article in journal (Refereed)
    Abstract [en]

Subtle nonverbal changes can influence perception, signal individual’s affective state and act as gateways in interpersonal communications. In this exploratory study, we investigated the effect of previously uninvestigated female leg posture (in-toeing vs. out-toeing) on gazing behavior and perceived attractiveness. Results showed a small effect: participants fixated more and spent more time looking at the legs and feet of the stimuli with in-toeing feet than parallel and out-toeing feet. Likewise, we found a small effect for the perception of the attractiveness. In line with the previous studies, we suggest in-toeing feet might signal femininity and submission and discussed our results accordingly.

Den "bästa" tiden?: En litteraturstudie baserad på självbiografier om mödrarrs upplevelser av postpartum depression2014Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

Domestic violence and perinatal outcomes - a prospective cohort study from Nepal2019In: BMC Public Health, ISSN 1471-2458, E-ISSN 1471-2458, Vol. 19, article id 671Article in journal (Refereed)
    Abstract [en]

Background: Domestic violence is one of the most common forms of violence against women. Domestic violence during pregnancy is associated with adverse perinatal and maternal outcomes. We aimed to assess whether domestic violence was associated with mode of delivery, low birthweight and preterm birth in two sites in Nepal. Methods: In this prospective cohort study we consecutively recruited 2004 pregnant women during antenatal care at two hospitals between June 2015 and September 2016. The Abuse Assessment Screen (modified) was used to assess fear and violence. Having ever experienced either fear or violence was defined as any domestic violence. Obstetric outcomes were obtained from hospital records for 1381 (69%) women, selecting singleton pregnancies only. Mode of delivery was assessed as birth by cesarean section or not. A birthweight of less than 2500g was defined as low birthweight and preterm birth as birth before completion of 37weeks gestation. Descriptive and multiple logistic regression analyses were performed to assess associations. Results: Twenty percent of the women reported any domestic violence. Among all 1381 women, 37.6% gave birth by cesarean section. Of those women who delivered by cesarean section, 84.7% had an emergency cesarean section. Less than 10% of the babies were born prematurely and 13.5% were born with low birthweight. We found no significant association between exposure to any domestic violence during pregnancy and risk of a low birthweight baby or birth by cesarean section. However, having experienced both violence and fear was significantly associated with giving birth to a preterm infant [aOR 2.33 (95% CI;1.10-4.73)]. Conclusions: Domestic violence is common in Nepal. This is a potential risk factor for severe morbidity and mortality in newborns. We found that the risk of having a preterm baby was higher for pregnant women who experienced both fear and violence. This should be recognized by the health sector. In this study, no significant differences were found in the rate of cesarean section nor low birthweight for women who had experienced any domestic violence compared to those who did not.

Cellulära förändringar i skelettmuskel inducerade av träning med fokus på PGC-1α: en systematisk litteraturstudie med fokus på PGC-1α och högintensiv intermittent träning2016Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

    Abstract

Background: Training conducted at high intensities appears to be a powerful method for improving aerobic performance and health, but the cellular mechanism behind these adaptations is still not fully understood. Animportant adaptation appears to be the mitochondrial biogenesis and a key protein linked to these musclecell changes seems to be PGC-1α.

It has long been known that endurance training increases the expression of PGC-1α, but more and more trainingmethods have been identified as increasing the expression of the protein. There is currently uncertainty regarding the exercise intensity that seems to be the most effective at increasing the expression of PGC-1α and mitochondrial biogenesis.

Purpose: The overall aim of this study was to study the muscular adaptation to training conducted in different intensity, with a focus on high-intensity intermittent training (HIIT) and the protein PGC-1α.

Method: This paper is a literature review. The paper is structured according to Goodman's seven steps model.

Results and Conclusion: Training conducted in an intensity corresponding to 100 % of VO2max generates most PGC-1α mRNA. Training conducted in supramaximal and submaximal intensity generates similar increases. It seems that the intermittent characteristics of the exercises are of less relevance to the expression of PGC-1α thus both continuous and interval workouts generate similar increases in protein. This is contrary to numerous studies' saying that high-intensity intermittent training at supramaximal intensity would be the most potent inducer of increased expression of PGC-1α mRNA.

An Internet based survey on how Swedish men who have sex with men percieve pre-travel prevention efforts on HIV/STIs2014Conference paper (Refereed)
    Abstract [en]

Background: Overseas travellers and men who have sex with men (MSM) are prioritised groups when it comes to HIV/STI prevention. Travelling increases sexually risky behaviour and sexual contacts abroad are common; among MSM, there is a high prevalence of HIV/STIs.

Objective: To investigate the experiences of and attitudes towards prevention efforts against HIV/STI before travelling abroad among MSM.

Methods: The study had a cross sectional design and is based on survey responses from 656 MSM, recruited through a Nordic website, who had travelled abroad during the preceding year of the survey. Analysis is primarily descriptive, but bivariate analyses were also performed, and statistically significant differences between groups were investigated using the chi-square test. The level of significance was p <. 05.

Results: Very few of the men felt that they encountered prevention efforts against HIV/STIs in Sweden (4.9%) and abroad (22.7%), and a majority (58.4%) felt that more should be offered, in particular to younger men. They felt that it was easy to find out information(79%), and most of them would use the Internet (87%). As to prevention efforts, a majority (68.3%) preferred free access to condoms and lubrications or written information/travel advice (59%).

Conclusion: Prevention efforts aimed at overseas travellers can involve links to information on the Internet about sexual health. The results suggest that it is primarily younger men who should be prioritised. It is important that the information is conveyed respectfully since this group may have the experience of feeling stigmatised or discriminated against.

HIV/STI prevention among young adults: a qualitative study on experiences of prevention efforts prior to travelling abroad2014Conference paper (Refereed)
    Abstract [en]

Background: Young adults are at risk for HIV/STIs because of multiple sexual partners and low condom use. Travelling increases sexually risky behaviour and sexual contacts abroad are common. Better knowledge about young adults’ experiences of prevention efforts prior to travelling as well as what they would prefer in terms of prevention efforts are important when planning prevention efforts targeted towards this group.

Objective: To investigate the experiences of and attitudes towards prevention efforts against HIV/STIs among young adults in Sweden who have travelled abroad.

Method: We conducted 12 focus-group interviews and four individual interviews with young adults (20–29 years) who had travelled abroad within the last 12-months, altogether 48 participants. The interviews were analysed using thematic content analysis.

Results: Only a few had any experience of prevention efforts against HIV/STIs. The majority welcomed the idea of prevention efforts prior to travelling abroad and would have welcomed more, preferably brief reminders, links to reliable websites or someone professional to discuss the issue with. Most of them would use the Internet to search for information. They proposed the possibility of reaching young adults through social media and the importance of receiving better basic knowledge in school.

Conclusion: Prevention efforts on HIV/STIs must focus on the use of established forums, when reaching young adults prior to trips abroad is difficult. Setting the foundation for a positive attitude towards condom use is needed during school years. Even social media, where there is the possibility for dialogue, should be used as an information source.

Totalt kaos men kraftfullt vackert: En litteraturstudie om patienters upplevelser av att leva med bipolär sjukdom och mötet med vården2018Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

Notification of an abnormal Pap smear: an intervention study2019In: European Journal of Cancer Care, ISSN 0961-5423, E-ISSN 1365-2354, Vol. 28, no 2, p. 1-7, article id e12969Article in journal (Refereed)
    Abstract [en]

This study aims to assess whether notification of an abnormal Pap smear result via a phone call, delivered by a trained healthcare provider, has an effect on women's HRQoL, coping and awareness of HPV. For this intervention study, women were consecutively recruited from a women's health clinic in Sweden. Women in the intervention group (n = 113) were notified of their Pap smear result via a phone call by a trained healthcare provider, while those in the comparison group (n = 122) were notified via a standard letter. A questionnaire was used to collect data. The results found no significant differences between the groups for HRQoL. However, 42.5% ofwomen in the intervention group versus 48.3% in the comparison group reported anxiety. Women in the intervention group were more satisfied with the manner in which they were notified of their abnormal result than those in the comparison group (92.0% vs. 67.2%; p < 0.001), more aware of HPV (71% vs. 50%; p = 0.001), and called healthcare services less often (10.6% vs. 18.0%; p = 0.113), no significance. In conclusion, notification of an abnormal Pap smear result via a phone call does not increase women's HRQoL or reduce their anxiety.

Swedish women's awareness of human papillomavirus, and health-related quality of life, anxiety and depression after a notification of an abnormal Pap smear result : a cross-sectional study2019In: European Journal of Cancer Prevention, ISSN 0959-8278, E-ISSN 1473-5709, Vol. 28, no 2, p. 96-101Article in journal (Refereed)
    Abstract [en]

This study aims to assess (a) women's awareness of the human papillomavirus (HPV), (b) women's health-related quality of life (HRQoL) and levels of anxiety and depression symptoms, and (c) to compare the outcomes between women who are aware of the sexually transmitted nature of the HPV infection and women who are not. Swedish women who have been notified of an abnormal Pap smear result completed a questionnaire. This questionnaire consisted of sociodemographic characteristics, items on awareness of HPV, and how to cope with the Pap smear result and the instruments: the Functional Assessment of Chronic Illness Therapy - Cervical Dysplasia and the Hospital Anxiety and Depression Scale (HADS). Data were analyzed using descriptive statistics, Student's t-test, χ-tests, Fisher's exact test, the Mann-Whitney U-test, and Fisher-Freeman-Halton exact test. In total, 122 women participated. The women reported a median (quartile 1-quartile 3) score of 87.6 on the Functional Assessment of Chronic Illness Therapy - Cervical Dysplasia (81.8-107.0), compatible with a good HRQoL. The median (quartile 1-quartile 3) scores on HADS-anxiety and HADS-depression were 7.0 (4.0-10.0) and 3.0 (1.0-5.3), respectively; however, 48.4% of the women reported anxiety (compared with 20% in a normal population). There were no statistically significant differences in the median scores in any of the scales, including the prevalence of distress between the subgroups. Women with abnormal Pap smear results have a good HRQoL; they can become anxious, but not depressed. Awareness of HPV as a sexually transmitted infection is low, but being aware does not impact on women's HRQoL or on anxiety and depression.

Understanding illnesses through a film festival: An observational study2019In: Heliyon, E-ISSN 2405-8440, Vol. 5, no 8, p. 1-8, article id e02196Article in journal (Refereed)
    Abstract [en]

Audio-visual materials play a fundamental role in the context of education, care and clinical treatment, as they seem to have a high impact on public awareness. This study aims to describe what messages are perceived by the society at an International Festival of Short Films and Art on Diseases that may help to understand difficult topics, such as illness, dying and suffering. Through an observational, descriptive, cross-sectional study, using full participant observation and an open, self-administered questionnaire, 32 short films were analysed during a healthcare art festival. Categories were developed using inductive content analysis. The message perceived by the participants, after the viewing of the shorts and reflection of the debates among the attendees, were considered in four categories: i) creative and positive education is possible; ii) awareness of preconception and practical duties; iii) meaning of life changes the experience of illness; iv) family and caregivers also experienced suffering. The short films are considered as an excellent tool to generate social dialogue and debate. Public events can be understood as an opportunity to acquire, in an emotional and critical manner, other competencies for public awareness. Together, they are capable of communicating difficult messages through a fast, positive, and creative way.

The paradox of being both needed and rejected: the existential meaning of being closely related to a person with bipolar disorder2012In: Issues in Mental Health Nursing, ISSN 0161-2840, E-ISSN 1096-4673, Vol. 33, no 4, p. 200-208Article in journal (Refereed)
    Abstract [en]

The aim of this study was to elucidate the existential meaning of being closely related to a person with bipolar disorder. A qualitative, descriptive and explorative design with a phenomenological meaning-oriented analysis was used. The findings reveal a paradoxical, existential exposure of close relatives to a person with bipolar disorder, being both needed and rejected whilst being overshadowed by the specific changeable nature of bipolar disorder. Psychiatric health care services are recommended to consider changes in attitudes and structures that may facilitate for close relatives` participation in the care and treatment of persons with bipolar disorder.

    Abstract [en]

Purpose: In this qualitative interview study we investigated the experiences of family members to cancer patients. Our objective was to explore and to differentiate their needs from the needs of cancer patients. Methods: Five focus groups and six individual narrative interviews with 17 family members to cancer patients in Sweden were conducted and compared with 19 cancer patient interviews. Our analysis was inspired by classic grounded theory. Results: Family members to cancer patients expressed own morbidity connected to high stress levels and difficulties in recognizing own stress due to ongoing comparisons with the cancer patient. Family members were trapped in a momentary terror-like situation where they became their sick relative's safety net. A percieved inability to improve their loved one's well being contributed to a feeling of guilt. The longing for it all to end was encumbered with shame since the end included possible death. Conclusions: By recognizing cancer as a disease striking both body and relationships, family members are given precedence over their own struggles, differentiated from the patient's experiences. We define differences in needs between cancer patients and family members. Family members to cancer patients may be supported in developing balancing strategies towards less stress, increased safety and moments of contentment.

Patients had several suggestions on how to improve healthcare meetings in order to create safer care

An open question "Do you have suggestions on how to improve meetings between patients and healthcare professionals in order to create safer care?" was incorporated into a questionnaire survey to 2673 patients in Sweden. The survey addressed patient participation for safer care. The open question was answered by 591 respondents. Content analysis was used to analyze the responses. The proposed suggestions concerned both the individual level (healthcare staff's competence and trust in the patient) and the system level (forms of communication, planning and structure, and time and staffing). The study findings show that there are many ways to improve meetings in healthcare of potential relevance for patient safety. Further research is needed to develop, apply and evaluate interventions based on patient suggestions.

Experiences of well-being among female doctoral students in Sweden2014In: International Journal of Qualitative Studies on Health and Well-being, ISSN 1748-2623, E-ISSN 1748-2631, Vol. 9, no 23059Article in journal (Refereed)
    Abstract [en]

The aim of this study was to explore how female PhD students experience and perceive their well-being. Focus groups were conducted with female PhD students employed at a Swedish university. The study was performed using a phenomenological hermeneutic approach based on the concept of the lifeworld, used as both a philosophical perspective and a methodology. Three main themes emerged from the analysis: being true to oneself, being in the sphere of influence, and performing a balancing act. By unfolding these themes, the study shows that perceptions and experiences of well-being in female PhD students are a multifaceted phenomenon and materialize through interaction of different aspects of ‘‘self ’’ (agent) and ‘‘others’’ (structure). As well as illustrating these perceptions and experiences, the study also presents female PhD students’ conceptualization of their well-being, expressed in terms of a white-water rafting metaphor.

Assessing Medical Students' Communication Skills by the Use of Standardized Patients: Emphasizing Standardized Patients' Quality Assurance2014In: Academic Psychiatry, ISSN 1042-9670, E-ISSN 1545-7230, Vol. 38, no 3, p. 354-360Article in journal (Refereed)
    Abstract [en]

The objective structured examination is one of the most valid, reliable, and effective tools for assessing clinical and communication skills, often by use of standard patients (SPs). SPs can also be assessors of those skills. One of the crucial areas when utilizing SP-based assessment is the quality and consistency assurance of their portrayal of the case and their ability to fill in checklists in an adequate way. The aim of this study was to assess the validity and reliability of SPs' ability to assess students' communication skill via a Calgary-Cambridge checklist. This cross-sectional and correlational study was conducted at the Tehran University of Medical Science. We first analyzed validity; the criterion validity of the SPs' filling in the checklists was assessed through determining the correlation between the SPs' completed checklists and the checklists filled in by three physician raters individually and then reproducibility: it was assessed by a test-retest approach inter-rater reliability. The mean correlation for assessing the validity of SPs' completed checklists by individual SPs was 0.81. The inter-rater reliability was calculated by kappa coefficient, and the total correlation among the three raters was 0.85. The reliability of the test-retest approach showed no significant differences between the test and re-test results. The increased number of medical students and different faculties' responsibilities such as doing educational, research, and health services duties assessing medical student communication skills is a complex issue. The results of our study showed that trained SPs can be used as a valid tool to assess medical students' communication skills, which is also more cost effective and reduces work load of medical faculties.

    Abstract [sv]

Doctors Doing Gender at Eye Clinics: Gender Constructions in Relation to Waiting Times for Cataract Extractions in Sweden2017In: NORA: Nordic Journal of Feminist and Gender Research, ISSN 0803-8740, E-ISSN 1502-394X, Vol. 25, no 2, p. 107-125Article in journal (Refereed)
    Abstract [en]

Why do eye clinics differ in their waiting times for women's and men's access to cataract extraction (CE)/gra starroperation? Taking a doing-gender perspective as the starting point, this study explores how gender constructions embedded in Swedish eye clinics contribute to longer waiting times for women than for men. Focus group interviews were conducted with doctors at two Swedish eye clinics: one with a larger and another with a smaller than average gender difference in waiting times for CE. Several differences were found between the clinics regarding how gender was constructed: Women and men were constructed as different with respect to ascribed traits such as assertiveness and care-seeking behaviour. Their need for visual acuity in working life was perceived as different by the doctors, and the study indicates differences between the clinics regarding their interest and awareness concerning issues related to inequity, reflected in the dissimilar prevalence of jokes with racist and misogynist connotations at the two clinics.

Digitalization of Patient Information Process from Hospital to Community (Home) Care Nurses: International Perspectives2018In: Nursing Informatics 2018: ICT to improve quality and safety at the point of care / [ed] Ann Kristin Rotegård, Diane J. Skiba, Sayonara Barbosa & Angelica G. Davalos Alcázar, IOS Press, 2018, Vol. 250, p. 227-229Conference paper (Refereed)
    Abstract [en]

For older people who transition from hospital to home, home care is an increasingly important and effective way of managing chronic illness with skilled nursing care in the home. Communication between clinicians across care settings is fundamental for continuity of care. Poor communication of patient information is acknowledged to be a root cause of sentinel events. In Scandinavia and the United States, researchers study the communication of patient information from hospital to home care. The health care systems of these two countries differ. Differences are reflected in the characteristics of patients transferred from hospital to home care, as well as the characteristics of health information systems (ICT). These differences help to elucidate key issues related to the transfer of patient information from hospital to home care.

Sjukskrivningskoordinerande sjuksköterskors erfarenheter av arbetet med att stärka sjukskrivnas självförmåga2015Independent thesis Advanced level (degree of Master (One Year)), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

Background: Sweden is one of the countries with the highest sick leave and in order to reduce it, with it´s consequences för the individual and the society, the Swedish government did a financial investment called Sjukskrivningsmiljarden. This increased the ability to, due to the coordinators function, increase the efficiency of patients’ rehabilitation process. Being on sick leave has consequences for patients in many areas, not least the quality of life. The length of sick leave is dependent upon patients´ self-efficacy concerning rehabilitation and returning to work. Research, such as Noordik et al. (2011), also indicates how patients´ self-efficacy can be strengthen by the support from the coordinator and thereby increase the capability to rehabilitation and return to work.

Aim: The aim of the study was to describe the sick leave-coordinating nurses’ experiences of strengthening self-efficacy in patients on sick leave.

Method: A qualitative approach was used and data was collected by semi-structured interwievs. Eight sick leave-coordinating nurses participated and interviews were analysed with manifest content analysis.

Results: Three categories resulted. The category Needs and conditions of the coordinator in order to strengthen the self-efficacy of the patient on sick leave describes the knowledge and factors needed to increase the patients self-efficacy. It is, among other things, important that the coordinator has knowledge in how the social insurance system works and that the coordinator uses her professional experiences as a nurse in contact with patients on sick leave. The relationship between the coordinator and the patient on sick leave describes the value of adopting a holistic perspective, to be available for the patient and the immanent risk of doing too much for the patient. The last category, Conducting the tasks of the coordinator, illustrates perceptions of how the coordinator conducts her work. Coordinators described how to motivate patients; how to support the patient to find comprehensibility; and how coordinators cooperate with other stakeholders.

Conclusion: Sick leave-coordinators emphasizes the importance of the self-efficacy of the patient on sick leave. A relationship based in thrust is needed to strengthen the patients self-efficacy. The holistic perspective proposed by the coordinators opens opportunitites to individually adapt each patient´s the process of rehabilitation and return to work.

A project to promote equality in ante-natal care for women who do not speak Swedish2017Conference paper (Other academic)
    Abstract [en]

    Background

Health care in Sweden aims to be equal. In antenatal care, women who do not speak Swedish have a higher rate of maternal and infant mortality (Essen et al. 2001, Wahlberg et al. 2013). Cultural and communicative problems have been identified as one reason for this disparity. Due to recent immigration, Arabic-speaking women (ASW) have been identified as a vulnerable group. Technology has the potential to solve language and cultural barriers (Haith-Cooper 2014). A pilot study in which midwives were interviewed suggested that an ‘app’ for a tablet with multimodal components could be an aid to cultural and linguistic understanding (Axelsson et al. 2016).

    Aim

The aim of the project is to identify women’s and midwives’ information needs and cultural issues, and subsequently, to develop an app with norm-critical design for interactive communication in antenatal care (ANC). The app will serve as a complement to interpreters and provide essential information about ANC.

    Method

Two data collection methods will be used. The first is to observe meetings between midwives and pregnant women. The second is to interview midwives in focus groups so that they can freely express their wishes. Following this, a trial version of the app will be constructed, which will then be tested and evaluated iteratively in midwifery practice. The material will be analysed in workshops using linguistic and norm critique methods.

    Results

The app is expected to improve and ensure the quality of ANC for ASW and improve their understanding of pregnancy, fetal development and parenting. It will hlep midwives to be sure that women have been given the correct information and improve patient safety. We will improve our own understanding of the risks in communication that can arise when two parties do not understand each other, and of how communication can be promoted through an app.

    Abstract [en]

Background: Rhythmical Einreibungen according to Wegman/Hauschka (RE) is a soft massage therapy for use in medicine and nursing extended through anthroposophy. RE is practiced by nurses working with integrative health care in the public financed health system, both with inpatients and outpatients (e.g. in primary care). The ontological definitions of caring science grounded in lifeworld theory of the human body, health and suffering is to a high extent corresponding with anthroposophic medicine.

Aim: To explore how outpatients receiving RE have experienced that the therapy have affected them.

Method: A quality content analysis, according to Lundman and Hällgren Graneheim (2012), based on semi structured interviews with five patients.

Results: A decrease in physical suffering such as pain, reduced lung capacity, IBS and sleeping disorders was experienced. Psychological and spiritual improvement was experienced in cognitive functions, sense of relaxation, meaning, hope, confirmation, security and vitality. Improved health was experienced despite severe illness.

Conclusion: RE is a therapy where caring science seems to be put in practice. This study on outpatients receiving RE indicates that it may improve health physically, psychologically and spiritually. The results match with earlier studies on inpatients receiving RE. Further studies, e.g. in comparison with psychotherapy or medical treatments, would be of great interest.

    Abstract [en]

Background: Nursing students can complete clinical training in educational hospital wards. A model called Developing and Learning Care Unit (ULVE), where students care and supports in pair. Studies show that the model supports student learning, but there are few studies about how patients perceive to be cared for by students in pairs. It would be of interest to examine what patient’s experience are, what they think about being cared of students who work in pairs and if they believe that they receive good care.

Aim: The aim of this study is to describe how patients’ experience being cared for by student nurses who are caring and supervised in pairs.

Method: The study was conducted with a reflective lifeworld approach based on phenomenological epistemology. The phenomenon of the study is ”to be cared for by  student nurses caring and supports in pairs”. Data was collected through interviews of seventeen patients cared for  within ULVE.

Results: The care of students who are supported in pairs means to become involved in student learning and to meet the responsibility and a desire to nurture and learn. Patients feel secure and are positive to be looked after by student nurses in pair.  Confidence in the healthcare provided under the supervision also contributes to security in the care situation. The result is further described with four meaningful elements: Being involved  in learning and caring, being the center of care,  confidence and trust in the quality of care and  getting something out of the ”ordinary”.

Conclusion: The study showed that patients cared by pairs of students is taken seriously and humanly during supervision of nursing clinician and it focuses in a patient centered care. This method of pair nursing students care shows respect, thoughtfulness, engagement and presence to patients where needs are fulfilled.

Prevalence of gender violence: studies of four kinds of abuse in five Nordic countries2003Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    Background

Abuse against women causes much suffering for the individual and is a major public health problem. The general aims of the present studies were I. to validate the NorVold Abuse Questionnaire (NorAQ) in a randomised population sample, and 2. to estimate the prevalence of emotional, physical and sexual abuse and abuse in the health care system by means of NorAQ in seven Nordic gynaecology and one Swedish population sample.

    Methods

In the validation study, data were collected in two steps. (I) NorAQ was sent to a random sample of 1923 women in Östergötland. (2) A subsample of 64 women filled in the NorAQ for a second time and were interviewed. The interview had open questions about abuse and was considered our gold standard. In the prevalence studies, NorAQ was sent to 6729 women visiting seven departments of gynaecology in Denmark (1), Finland (1), Iceland (1), Norway (1) and Sweden (3). The main outcome measures were prevalence rates of emotional, physical, and sexual abuse and abuse in the health care system, current suffering from abuse and cmmmmication of a history of abuse to the gynaecologist.

    Findings

The 13 questions in NorAQ concerning experiences of abuse had satisfactory validity and reliability. The response rate was 67-85% at the clinics and 61% in the Swedish population sample. In general the participants did not feel uncomfortable when answering NorAQ. We found differences in lifetime prevalence of the four kinds of abuse as defined by NorAQ among the Nordic cmmtries: emotional abuse 19-37%; physical abuse 38-66%; sexual abuse 17-33%; abuse in the health care system 13-28%. In Sweden, the lifetime prevalence of abuse was in clinical and population samples: emotional abuse 17-21 %; physical abuse 32-38%; sexual abuse 16-17%; abuse in the health care system 14-20%. There were generally no differences in prevalence rates of abuse among the four Swedish samples, except for abuse in the health care system, which was more commonly reported in the clinical sample of Linköping than in the population sample of Östergötland. Not all abused women reported current suffering from the abusive experience. To estimate prevalence of such abuse that causes current suffering is a new approach. Childhood experiences of emotional, physical and/or sexual abuse were associated with adult experiences of abuse in the health care system. Yet two thirds of women who felt abused in the health care system as adult had no history of earlier abuse. Most women had not talked to the gynaecologist about their experiences of abuse at their latest visit to the clinic.

    Conclusion

The questions about abuse in NorAQ had good validity and reliability. We found high prevalences of all four kinds of abuse. Prevalence estimates are highly sensitive to methodology issues and must be related to a specified definition of abuse. Prevalence of abuse causing current suffering was considerably lower than the mere occurrence rates for all fom kinds of abuse. In general, women had not discussed a background of abuse with the gynaecologist at their latest visit to the clinic. Experiences of emotional, physical and/or sexual abuse in childhood were associated with adult abuse in the health care system. Yet the majority of our cases were not victims of childhood abuse. Both findings call for attention and further exploration.

Can Forum Play Contribute to Counteracting Abuse in Health Care?: A Pilot Intervention Study in Sri Lanka2019In: International Journal of Environmental Research and Public Health, ISSN 1661-7827, E-ISSN 1660-4601, Vol. 16, no 9, p. 1-10, article id 1616Article in journal (Refereed)
    Abstract [en]

Obstetric violence refers to the mistreatment of women in pregnancy and childbirth care by their health providers. It is linked to poor quality of care, lack of trust in health systems, and adverse maternal and neonatal outcomes. Evidence of interventions to reduce and prevent obstetric violence is limited. We developed a training intervention using a participatory theatre technique called Forum Play inspired by the Theatre of the Oppressed for health providers in Sri Lanka. This paper assesses the potential of the training method to increase staff awareness of obstetric violence and promote taking action to reduce or prevent it. We conducted four workshops with 20 physicians and 30 nurses working in three hospitals in Colombo, Sri Lanka. Participants completed a questionnaire before and three-to-four months after the intervention. At follow-up, participants more often reported that they had been involved in situations of obstetric violence, indicating new knowledge of the phenomenon and/or an increase in their ability to conceptualise it. The intervention appears promising for improving the abilities of health care providers to recognise obstetric violence, the first step in counteracting it. The study demonstrates the value of developing further studies to assess the longitudinal impacts of theatre-based training interventions to reduce obstetric violence and, ultimately, improve patient care.
