    Abstract [en]

    Objective

    To evaluate Swedish pharmacists’ attitudes towards ePrescribing, including thetransfer of ePrescriptions, electronic storing of prescriptions and mail-order prescriptions.

    Method

    This study was conducted as a web-based survey among 500 randomly selectedSwedish community pharmacists. The questionnaire included open-ended questions,multiple-choice questions and statements to which respondents gave their degree ofagreement on a six-point Likert-type rating scale. Free-text answers were categorized intothe most commonly mentioned opinions. Differences in respondents’ general characteristicsand their level of satisfaction with ePrescribing were tested for significance using χ2test. The survey had a response rate of 52% (259/500). 96% (248/259) of the respondentswere female, and 76% (196/259) were aged 40–64 years.

    Key findings

    Swedish community pharmacists were generally satisfied with ePrescribing(98%, 253/259). No significant (P > 0.05) difference in the general satisfaction could beexplained by the respondents’ age, gender or years in practice. A majority of the respondentsregarded ePrescriptions and electronic storing of prescriptions as being safe for patients (95 and93% respectively), providing patient benefits (96 and 95% respectively), being cost-effectivefor the pharmacy (92 and 91% respectively) and contributing to better communication andrelationships with both patients and prescribers (62–88%). The positive aspects of ePrescribingmost frequently mentioned in free-text answers were being safe (72%, 187/259) and timesaving(55%, 143/259). However, several weaknesses with ePrescribing were also describedby the respondents, as well as suggestions for improvement.

    Conclusion

    Our nationwide survey of Swedish community pharmacists’ attitudestowards ePrescribing shows that pharmacists are generally satisfied with it, including thetransfer of ePrescriptions, electronic storing of prescriptions and to a lower degree mailorderprescriptions.

    Abstract [en]

    Objective

    To evaluate Swedish patients’ attitudes towards e-prescribing, including the transfer of e-prescriptions, electronic storing of prescriptions and mail-order prescriptions.

    Methods

    This study was a nationwide survey of attitudes among Swedish patients and was conducted as a postal questionnaire. The questionnaire was developed for the purpose of thisstudy and aimed to evaluate respondents’ views concerning e-prescribing, electronic storingof e-prescriptions and mail-order prescriptions from aspects including safety, personal benefits and effectiveness. A study population of 1500 individuals meeting the inclusioncriteria was randomly selected from a database of individuals in Sweden storing prescriptions electronically (n = 5 840 599).The response rate was 52% (739/1429).

    Key findings

    The vast majority of the respondents had a positive attitude towards e-prescriptions (85%, 628/739) and electronic storing of prescriptions (86%, 633/739), andregarded e-prescriptions to be safe (79%, 584/739), creating benefits for them (78%, 576/739) and promoting faster dispensing (69%, 512/739). Significant differences in attitudes towards e-prescriptions and electronic storing of prescriptions were detected between age groups. Patients storing all their prescriptions electronically had a more positive attitudetowards both e-prescriptions and electronic storing of prescriptions compared to patientswho stated they had paper prescriptions. The most common suggestion (n = 27) for improvement was to extend the information given about the services.

    Conclusion

    Our nationwide survey showed that a vast majority of Swedish patients had positive attitudes towards e-prescriptions and electronic storing of prescriptions. However, a need for extended information regarding e-prescribing was identified.

    Abstract [en]

    With their efficiency and nano-scaled size, cytoskeletal motors propose a promising addition to new technical innovations. These small workhorses are perfected by evolution and work with high proficiency in nanoscale environments. Since they utilize ATP as energy source they are rendered independent of an external power source. This opens up for the development of advanced mobile diagnostic devices that are as easy to use as a home-use pregnancy test. If they could deliver attached cargo molecules uni-directionally to different reaction chambers on lab-on-a-chip devices, they could replace the use of microfluids. This would include using topographically designed nanowire surfaces, such as aluminum oxide. This paper focuses on the design of a non-etching surface treatment for these gallium phosphate surfaces, which yield a high motility while yet being easy to reproduce. A step-by-step experiment was constructed and involves the use of various surface-cleaning and silanization procedures. The surfaces chemistry and morphology was analyzed with contact angle measurements, ellipsometry as well as in vitro motility assays. While a thought to be favorable surface chemistry was successfully attained utilizing 18 M sulphuric acid together with a following silanization using TMCS, a high motility was not. It was speculated that further manipulation of the silanization protocol and its chemicals could yield a more satisfactory result.

    Although wood floor production does not use water in the production process, water consumptionis related to cleaning and washing of floor and machineries in different steps of the process line,which generate a number of small flows that are highly polluted.Besides this, the industry has a need to store large amounts of wood outside to be able to havecontinuity in the production. This takes up a lot of space outdoors and once it rains the water thathas been in contact with wood, oil and metals forms stormwater, which transports pollutants.Stormwater has for a long time not been seen as a problem and has often been discharged intorecipient water bodies without any treatment. During cold seasons, this also involves snowmelt thatcan transport high concentrations of different pollutants.This report describes the composition of process- and stormwater from a wood floor industry inNybro, Sweden regarding parameters such as COD, phenol, tannin and lignin. The concentrationsof phenols in the stormwater were found in a range considered toxic to marine life.Regarding the process water, high values was found for COD (Chemical Oxygen Demand) and forother substances and elements potentially toxic (e.g. formaldehyde, wood resins, detergents andmetals). If these waters are directly released to a sewage treatment plant without any pre-treatmentprocess it can disturb the plant treatment efficiency; if released to a recipient water body, it cancause oxygen deficiency and consequently, death to marine life.The possibility of reducing the levels of pollutants through the use of electrocoagulation has beenexamined in this study. This has been done both for process water and stormwater from the woodfloor industry. A 250 ml batch unit for electrocoagulation EC was setup with iron (Fe) andaluminium (Al) electrodes for treating process water and stormwater. The results show that the ECprocess can reduce COD concentration from stormwater at least 70%. On the other hand, lessefficiency of EC for treating process water was observed.A method for simulating a snowmelt period in lab scale was also developed. Snow collected from awood floor industry was melted according to real temperature and the quality of these samples hasthen been compared to on-site samples of stormwater

    There is a need for the development of on-site wastewater treatment technologies suitable for "dry-process industries," such as the wood-floor sector. Due to the nature of their activities, these industries generate lower volumes of highly polluted wastewaters after cleaning activities. Advanced oxidation processes such as Fenton and photo-Fenton, are potentially feasible options for treatment of these wastewaters. One of the disadvantages of the Fenton process is the formation of large amounts of ferrous iron sludge, a constraint that might be overcome with the use of nanoscale zero-valent iron (nZVI) powder. Wastewater from a wood-floor industry with initial COD of 4956 mg/L and TOC of 2730 mg/L was treated with dark-Fenton (nZVI/H2O2) and photo-Fenton (nZVI/H2O2/UV) applying a 2-level full-factorial experimental design. The highest removal of COD and TOC (80% and 60%, resp.) was achieved using photo-Fenton. The supply of the reactants in more than one dose during the reaction time had significant and positive effects on the treatment efficiency. According to the results, Fenton and mostly photo-Fenton are promising treatment options for these highly recalcitrant wastewaters. Future investigations should focus on optimizing treatment processes and assessing toxic effects that residual pollutants and the nZVI might have. The feasibility of combining advanced oxidation processes with biological treatment is also recommended.

    Syfte: Denna undersökning utreder om polariserande Suncovers skulle kunna påverka djupseendet.

    Metod: Mätningarna genomfördes på tre meters avstånd med en modifierad variant av Frisby Davis Distance Stereotest och med jämförande mätningar mellan bruna Suncovers från Polaroid, gråa Suncovers från Polaroid och utan solskydd. Undersökningarna gjordes på 36 frivilliga försökspersoner som alla hade samsyn och inte mer visusskillnad mellan ögonen än 0,1 med en synskärpa över 0,9 Snellen-visus.

    Resultat: Utan solglasögon var medelvärdet på stereoseendet för försökspersonerna 16,0´´ ± 10,7´´. Med polariserande Suncovers blev det en försämring, denna försämring var 2,8´´ med gråa Suncovers (p = 0,08) och 5,4´´ med bruna Suncovers (p = 0,02).

    Slutsats: Med de bruna Suncovers fanns en signifikant försämring i jämförelse med utan Suncovers även om skillnaden var liten. Att det är en större påverkan med bruna Suncovers än med gråa kan möjligtvis bero på att de bruna till skillnad mot de gråa absorberar våglängderna ojämnt över det synliga spektrumet.

    Abstract [sv]

    Detta examensarbete gick ut på att ta fram en metod med hjälp av en texturmätare där olika kvalitetsparametrar såsom hårdhet, elasticitet och spänst kunde mätas på pasta. Dessa mätningar gjordes på tre olika spaghettimärken. Dessa var Kungsörnen, Tomadini samt De Cecco. Mätningarna på de olika pastamärkena gjordes med hjälp av en texturmätare från TexVol som är ett svenskt företag. Deras olika instrument är främst framtagna för att fungera inom bageribranchen och köttindustrin. Detta gjorde att det var spännande att se om detta instrument även kunde användas med framgång för att mäta olika kvalitetsegenskaper på kokt pasta. Först gjordes mätningar på Kungsörnens spaghetti vid olika koktider. Dessa var mellan 5 och 14 minuter. Dessa mätningar gjordes främst för att se vad som händer med de olika kvalitetsegenskaperna från det att pastan är underkokt till kraftigt överkokt. Utifrån resultaten från dessa mätningar är det svårt att dra några tydliga slutsatser då de varierade kraftigt. Därefter gjordes mätningar på de tre olika pastamärkena med två olika mätmetoder, dels single cycle och dels hold until time. Med single cycle menas att proben(mätstången) går ner i pastan och sen direkt upp igen medan vid hold until time metoden så går proben ner för att sen stanna nere tills en förinställd tid passerat innan den går upp igen. På detta sätt kan olika kvalitetsegenskaper mätas med de olika mätmetoderna. För att dessa mätmetoder ska fungerar har olika parametrar på texturmätaren utvärderats och på detta sätt har en metod/tillvägagångssätt tagits fram. De flesta av stegen i denna metod verkar fungera bra men ett stort problem är att mätningarna skiljer sig mycket åt. Detta beror troligtvis på att pastan lägger sig olika inför varje mätning vilket gör att mätningarna blir långt ifrån identiska vilket leder till att resultaten blir svårtolkade. Vissa tendenser kan dock ändå ses. Om alla mätningar tags i beaktande kan det sägas att De Cecco pastan är den pasta av högst kvalité. De Cecco pastan är den som är mest "al dente", mest elastisk, mest spänst samt lägst klibbighet. Vad detta beror på är svårt att säga då det kan finnas många förklaringar som är mer eller mindre relevanta. En trolig förklaring kan vara att De Cecco pastan har ett högre proteininnehåll än de båda andra märkena. I litteraturen står det att ett högre proteininnehåll ger en bättre kokt pasta och detta verkar rimligt då protein nätverket blir starkare vilket håller ihop pastan på ett bättre sätt. En annan möjlig orsak kan vara att fördelningen av de olika proteinerna är bättre i De Cecco pastan än i de andra märkena vilket leder till en bättre kvalitet. De två andra, Kungsörnen och Tomadini hade snarlika resultat.

    Vad som behöver göras för att denna metod eventuellt ska kunna användas i den dagliga produktionen av pasta och utveckling av ny pasta är att ta bort så många felkällor som möjligt så att standardavvikelsen i alla mätningar blir avsevärt mindre. Detta kan kanske avhjälpas av att ha en mjukvara som är mer anpassad till pastamätningar än den som var används nu som är anpassad till bageribranchen. En annan viktig sak kan vara att mäta på bara några få pasta strå till skillnad mot nuvarande "spaghettinystan". Ytterligare en intressant sak är att göra en organoleptisk undersökning av de olika pastamärkena för att se om de uppmätta resultaten är smakbara. Även att titta i elektronmikroskåp för att se de olika spaghettis proteinnätverk kan vara intressant för att få en förståelse för hur pastan ändrar sig under kokning.

    Antibiotic resistance in avian bacterial pathogens is a common problem in the Bangladesh poultry industry. The aim of the present study was to provide information on the present status of antibiotic resistance patterns in avian pathogenic Escherichia coli in Bangladesh. Of 279 dead or sick poultry of different ages, 101 pathogenic E. coli strains isolated from broilers and layer hens with colibacillosis infections were screened to determine phenotypic expression of antimicrobial resistance against 13 antibiotics used in both veterinary and human medicine in Bangladesh. Of 101 pathogenic E. coli isolates, more than 55% were resistant to at least one or more of the tested compounds, and 36.6% of the isolates showed multiple-drug-resistant phenotypes. The most common resistances observed were against tetracycline (45.5%), trimethoprim-sulphamethoxazole (26.7%), nalidixic acid (25.7%), ampicillin (25.7%), and streptomycin (20.8%). Resistance to ciprofloxacin (12.9%), chlormaphenicol (8.9%), nitrofurantoin (2%), and gentamicin (2%) was also observed, and none of the isolates were resistant to tigecycline as well as extended spectrum beta-lactamase (ESBL) producers. One isolate was resistant to cefuroxime (1%), cefadroxil (1%), and mecillinam (1%) but was not an ESBL producer. Resistance rates, although significant in Bangladeshi isolates, were found to be lower than those reported for avian isolates from the Republic of Korea and clinical, avian, and environmental isolates from Bangladesh. The high level of antibiotic resistance in avian pathogens from Bangladesh is worrisome and indicates that widespread use of antibiotics as feed additives for growth promotion and disease prevention could have negative implications for human and animal health and the environment.

    Multidrug resistance was found in 22.7% of Escherichia coli isolates from bird samples in Bangladesh; 30% produced extended-spectrum β-lactamases, including clones of CTX-M genes among wild and domestic birds. Unrestricted use of antimicrobial drugs in feed for domestic birds and the spread of resistance genes to the large bird reservoir in Bangladesh are growing problems.

    Sustainable architecture is about meeting the environmental, cultural, social and economical needs of today without compromising the ability of future generations to fulfill their own needs. This is quite a complex and challenging task because the architect must consider the impact of these needs through the whole life cycle of the building.

    The objective of this thesis is to evaluate what the management can do to support and create an environment were the architects can inspire and excel in the complex and challanging task of implementing sustainable solutions in architectural projects. Further to map prohibiting factors that architects working with sustainable architecture meet and describe the whole building chain and what factors are important to support development of sustainable architecture.

    This thesis investigates in theories of environmental management, knowledge management, change management, psychological and economical aspects of environmental problems. The theories draws among some principal guidelines that the management should aspire to define a new business logic that is rooted in a wider view of human values and ethics, concerning time, quality, cost reduction, distribution and critical staffing issues. Encourage employees to respond emotionally to an issue is a natural and powerful motivation for action. They see, then feel and then change behavior. Encourage employees to spend time in the nature to experience that we are interconnected with the nature and that we are dependent on it. This perspective leads to sustainable behavior that is not based on self-sacrifice or self-denial, but out of a sense of love and common identity.

    The architectural organizations interviewed describes quite similar aspects but highlights the importance of letting environmental issues be equally important and incorporated with other significant issues, learning by doing and letting the process be flexible, take its time and give room for communication in order to develop naturally. When working with complex sustainable solutions it is important to avoid pre-accepted solutions by understanding the whole picture and thinking on your own. They experience how valuable it is to use successful environmental architecture as inspiration and how counterproductive “bad” environmental architecture is to sustainable development. The management must focus on removing prohibiting factors to relieve and protect the architects from these strains so that they can preserve and use their energy on sustainable development instead. Initiating projects with a broad approach, establishing a broad and qualified network and cooperating and communicate across the whole building chain in the initial stages of the project, supports better and holistic solutions and decrease the costs and time frame of the project. Overall one of the most important factors is to encouraging a feeling of community in the organization. It is central because it provides a work environment where the employees inspire, share knowledge and support each other to strive for good results. Such a work environment equips the organization to be better at facing rapidly changing and complex demands.

    Abstract [en]

    The relaxin/insulin superfamily is a group of peptide hormones that consists of ten members in human, namely relaxins 1-3, insulin-like peptides (INSL) 3-6, insulin and insulin-like growth factors (IGF) I-II. These peptides have various functions in the body, such as regulating growth, blood glucose levels,  collagen metabolism, germ cell maturation and appetite. Misregulation of these mechanisms is associated with disease and accordingly they are of interest as potential pharmaceutical targets. Structurally the hormones are characterised by two peptide chains, A and B, which are held together by one intra A-chain and two inter chain disulfide bonds. Four different G-protein coupled receptors (GPCR) called relaxin family peptide receptor (RXFP) 1-4 have been found to respond to stimuli by different relaxin peptides. RXFP3 and RXFP4 are classic peptide ligand GPCRs, whereas RXFP1 and RXFP2 are characterised by a large extracellular leucine rich-repeat domain. Relaxin-3, which is the relaxin family ancestor, is the only relaxin peptide known to be able to bind and activate both subtypes of GPCRs, namely RXFP1, RXFP3 and RXFP4.

    The aim of this thesis was to analyse the structure-function relationship of the relaxin ligands and receptors, and to use this information to develop selective ligands for the relaxin receptors, which can be used as drug leads or pharmacological tools for investigating the physiological roles of the RXFPs.

    The 3D structures of native INSL5 and relaxin-2 were determined by solution NMR spectroscopy. The peptides showed an insulin/relaxin-like overall fold. A relaxin chimera peptide, consisting of the A-chain from INSL5 and the B-chain from relaxin-3, R3/I5, which has been shown to be selective for RXFP3 and RXFP4 over RXFP1, was also subjected to NMR studies. The R3/I5 peptide maintained an insulin/relaxin-like overall fold, and the relaxin-3 B-chain adopted a conformation identical to that in native relaxin-3, confirming that the activity of R3/I5 can be directly related to its primary sequence. Furthermore, a truncation study was undertaken to ascertain the importance of the termini for structure and function. By using the knowledge generated from the structure-function relationship, a single-chain high affinity RXFP3 selective antagonist was developed.

    In conclusion, this thesis has contributed to broaden the knowledge of the structure-function relationship of the relaxin ligands and the development of a selective RXFP3 antagonist, which is currently a drug lead for treatment of neurological disorders including stress and obesity.

    Abstract [en]

    Background: The purpose of drug treatment is to reduce morbidity and mortality, and to improve health-related quality of life. However, there are frequent problems associated with drug treatment, especially among the elderly. The aim of this thesis was to investigate the impact of clinical pharmacy services within a multiprofessional healthcare team on quality and safety of patients’ drug therapy, and to study the frequency and nature of medication history errors on admission to hospital.

    Methods: A model for clinical pharmacy services within a multiprofessional healthcare team (the Lund Integrated Medicines Management model, LIMM) was introduced in three hospital wards. On admission of patients to hospital, clinical pharmacists conducted medication reconciliation (i.e. identified the most accurate list of a patient’s current medications) to identify any errors in the hospital medication list. To identify, solve and prevent any other drug-related problems, the clinical pharmacists interviewed patients and performed medication reviews and monitoring of drug therapy. Drug-related problems were discussed within the multiprofessional team and the physicians adjusted the drug therapy as appropriate.

    Results: On admission to hospital, drug-related problems, such as low adherence to drug therapy and concerns about treatment, were identified. Different statistical approaches to present results from ordinal data on adherence and beliefs about medicines were suggested. Approximately half of the patients were affected by errors in the medication history at admission to hospital; patients who had many prescription drugs had a higher risk for errors. Medication reconciliation and review reduced the number of inappropriate medications and reduced drug-related hospital revisits. No impact on all-cause hospital revisits was demonstrated.

    Conclusion: Patients admitted to hospital are at high risk for being affected by medication history errors and there is a high potential to improve their drug therapy. By reducing medication history errors and improving medication appropriateness, clinical pharmacy services within a multiprofessional healthcare team improve the quality and safety of patients’ drug therapy. The impact of routine implementation of medication reconciliation and review on healthcare visits will need further evaluation; the results from this thesis suggest that drug-related hospital revisits could be reduced.

    Abstract [en]

    Background: An accurate medication list at hospital admission is essential for the evaluation and further treatment of patients. The objective of this study was to describe the frequency, type and predictors of errors in medication history, and to evaluate the extent to which standard care corrects these errors.

    Methods:A descriptive study was carried out in two medical wards in a Swedish hospital using Lund Integrated Medicines Management (LIMM)-based medication reconciliation. A clinical pharmacist identified each patient's most accurate pre-admission medication list by conducting a medication reconciliation process shortly after admission. This list was then compared with the patient's medication list in the hospital medical records. Addition or withdrawal of a drug or changes to the dose or dosage form in the hospital medication list were considered medication discrepancies. Medication discrepancies for which no clinical reason could be identified (unintentional changes) were considered medication history errors.

    Results: The final study population comprised 670 of 818 eligible patients. At least one medication history error was identified by pharmacists conducting medication reconciliations for 313 of these patients (47%; 95% CI 43-51%). The most common medication error was an omitted drug, followed by a wrong dose. Multivariate logistic regression analysis showed that a higher number of drugs at admission (odds ratio [OR] per 1 drug increase = 1.10; 95% CI 1.06 - 1.14; p<0.0001) and the patient living in their own home without any care services (OR1.58; 95% CI 1.02 - 2.45; p = 0.042) were predictors for medication history errors at admission. The results further indicated that standard care by non-pharmacist ward staff had partly corrected the errors in affected patients by four days after admission, but a considerable proportion of the errors made in the initial medication history at admission remained undetected by standard care (OR for medication errors detected by pharmacists' medication reconciliation carried out on days 4 - 11 compared to days 0 - 1 = 0.52; 95% CI 0.30 - 0.91; p = 0.021).

    Conclusions: Clinical pharmacists conducting LIMM-based medication reconciliations have a high potential for correcting errors in medication history for all patients. In an older Swedish population, those prescribed many drugs seem to benefit most from admission medication reconciliation.

    Introduction Inappropriate prescribing among elderly patients may increase the risk for drug-related problems. The objective of this study was to examine the impact of the Lund Integrated Medicines Management (LIMM) model on medication appropriateness in hospitalised patients. Materials & Methods A prospective controlled study of 210 elderly patients, from three internal medicine wards in a Swedish hospital. We compared patients receiving Medication Reviews, including Admission Medication Reconciliation, according to the LIMM-model, with patients receiving standard care. The LIMMmodel was implemented at one ward at a time between January 2007 and March 2008. We chose four dates for inclusion of patients during this period, one date before implementation and three dates about one month after implementation on the first, second and third ward, respectively. All patients who were staying at any of the wards on  these dates were eligible for inclusion. The main outcome measure was the change, from admission to discharge, in the number of drugs with at least one inappropriate rating, according to the Medication Appropriateness Index (MAI). The MAI consists of 10 criteria: indication, effectiveness, dosage, correct directions, practical directions, drug-drug interactions, drug-disease interactions, duplication, duration and expense. For intervention patients, the MAI assessment at discharge was performed in two steps. Step 1, which was a blinded assessment based on information in the patient’s medical record, was used in the main analysis. In step 2, the drugs were reassessed (unblinded), by adding information from the pharmacists’ Medication Review Forms that had not been documented in the medical records. Results The control and intervention group were similar regarding baseline demographic data. The mean number of drugs with at least one inappropriate rating decreased from 3.0 at admission to 1.5 at discharge in the intervention group, and from 2.8 to 1.7 in the control group. This represented a 51% (95% CI, 43-58%) decrease in the intervention group (step 1 assessment), compared to a 39% (95% CI, 30-48%) decrease in the control group (p = 0.0446, Poisson regression). In the assessment step 2, the MAI rating was changed from ‘inappropriate’ to ‘appropriate’ in 23% of the drugs (27/117). The mean decrease per patient between step 1 and 2 in number of drugs with at least one inappropriate rating was 0.38 (95% CI 0.22–0.54). In both control and intervention patients the most common MAI problems involved ‘expense’, ‘indication’ and ‘duration’. Discussion & Conclusion This study showed that medication appropriateness at discharge from hospital improved in patients receiving Medication Reviews and Reconciliation, according to the LIMM-model. In order to take full advantage of the work done by the clinical pharmacists, the information documented in the Medication Review Forms should also be documented in the medical records.

    Purpose

    To examine the impact of systematic medication reconciliations when admitted to hospital, and medication review while in hospital, on the number of inappropriate medications and unscheduled drug-related hospital revisits in elderly patients.

    Methods

    A prospective, controlled study in 210 patients, aged 65 years or older, who were admitted to one of three internal medicine wards at a University Hospital in Sweden. Patients received either standard care or care according to the Lund Integrated Medicines Management (LIMM) model. A multi-professional team, including a clinical pharmacist, provided medication reconciliations on admission and medication reviews during the hospital stay for the LIMM group. Blinded reviewers evaluated the appropriateness of the prescribing (using the Medication Appropriateness Index) on admission and discharge, and assessed the probability that a drug-related problem was the reason for any patient readmitted to hospital or visiting the emergency department within three months of discharge (using WHO causality criteria).

    Results

    There was a greater decrease in the number of inappropriate drugs in the intervention group than in the control group for both the intention-to-treat population (51% [95% CI 43-58%] versus 39% [95% CI 30-48%], p=0.0446) and the per-protocol population (60% [95% CI 51-67%] versus 44% [95% CI 34-52 %], p=0.0106). There were 6 revisits to hospital in the intervention group which were judged as ‘possibly, probably or certainly drug-related’, compared with 12 in the control group (p=0.0469).

    Conclusion

    In this study, medication reconciliation and reviews provided by a clinical pharmacist in a multi-professional team significantly reduced the number of inappropriate drugs and unscheduled drug-related hospital revisits for elderly patients.

    What is known and objective: Interventions involving medication reconciliation and review by clinical pharmacists can reduce drug-related problems and improve therapeutic outcomes. The objective of this study was to examine the impact of routine admission medication reconciliation and inpatient medication review on emergency department (ED) revisits after discharge. Secondary outcomes included the combined rate of post-discharge hospital revisits or death.

    Methods: This prospective, controlled study included all patients hospitalised in three internal medicine wards in a university hospital, between January 1 2006 and May 31 2008. Medication reconciliation on admission and inpatient medication review, conducted by clinical pharmacists in a multiprofessional team, were implemented in these wards at different times during 2007 and 2008 (intervention periods). A discharge medication reconciliation was undertaken in all the study wards, during both control and intervention periods. Patients were included in the intervention group (n=1216) if they attended a ward with medication reconciliation and review, whether they had received the intervention or not. Control patients (n=2758) attended the wards before implementation of the intervention. 

    Results: No impact of medication reconciliation and reviews on ED revisits (hazard ratio [HR], 0.95; 95% confidence interval [CI], 0.86-1.04) or event-free survival (HR, 0.96; 95% CI, 0.88-1.04) was demonstrated. In the intervention group, 594 patients (48.8%) visited the ED, compared to 1416 (51.3%) control patients. In total, 716 intervention (58.9%) and 1688 (61.2%) control patients experienced any event (ED visit, hospitalisation or death). Because the time to a subsequent ED visit was longer for the control as well as the intervention groups in 2007 than in 2006 (p<0.05), we re-examined this cohort of patients; the proportion of patients revisiting the ED was similar in both groups in 2007 (p=0.608).

    What is new and conclusion: Routine implementation of medication reconciliation and reviews on admission and during the hospital stay did not appear to have any impact on ED revisits, rehospitalisations or mortality over six months' follow-up.

    Abstract [en]

    Erythropoiesis is a complex and carefully controlled process which is mainly regulated by erythropoietin. Using flow cytometry cells in a blood sample can be analyzed one by one. ADVIA 2120 using flow cytometry for analysis of red blood cells (RBC) and reticulocytes. Red blood cell distribution width (RDW) is a component of the RBC method. RDW indicates size variation on erythrocytes and is measured as a percentage of the mean cell volume (MCV). By measuring the RDW anemia can be divided into makrocytic, microcytic or normocytic. Reticulocytes refract light more strongly than red blood cells because they contain some cellular debris. The number of reticulocytes in the peripheral blood reflects erythropoiesis in the bone marrow. 

    The purpose of this project was that by approved methods to establish reference ranges for the parameters, RDW and reticulocyte counts.

    The individuals who participated in the study, n=124, were considered healthy and had varying ethnicity. Reference subjects had not donated blood the last six months and participation in the study was voluntary.

    The statistic evaluation includes examination of distribution, and reference ranges for both men and women were established using the 2.5:t and 97.5:t percentiles. The calculated reference interval for RDW was between 12.0 – 14.3% and the calculated reference interval for reticulocytes was between 38.0 – 112.8 x109 cells/L. The results showed a significant difference between males and females for the parameter reticulocytes was seen. The difference indicates that different reference ranges should be established for men and women (p<0,0001). No significant difference between males and females were seen for RDW (p=0,74) indicating that the same reference ranges can be used for both men and women.

    Insights into the Isomerisation Mechanism of Warfarin2010Konferensbidrag (Refereegranskat)
    Abstract [en]

    Warfarin is one of the most commonly used drugs in anticoagulent therapy. Notwithstanding its wide use, achieving correct dosage is often a major challenge due to its narrow therapeutic window.[1] The bioavailability of warfarin is believed to be greatly influenced by the environment-dependent composition of the ensemble of isomers present. While the different structures of warfarin have been discussed in earlier publications,[2] details of the mechanism underlying the formation of the cyclic hemiacetal (Figure 1) had not yet been investigated.

    We have now studied the reaction by means of density functional calculations. Comparison of results from calculations performed on the isolated warfarin molecule and in presence of water molecules (compare Figure 2) highlight the importance of intermolecular interactions in the key proton transfer step for the reaction to proceed. A viable model for the mechanism underlying the isomerisation shall be presented.

    Abstract [en]

    The anticoagulent drug warfarin exhibits chameleon-like isomerism, where the environment-dependent composition of the ensemble of structures greatly influences its bioavailability. Here, the mechanism of conversion between the major isomeric forms is studied. The dramatic differences in transition state energies, as determined by density functional calculations, highlight the necessity for the involvement of intermolecular interactions in the key proton transfer step. A viable model for the mechanism underlying the isomerization reactions is presented.

    Here we present a series of molecularly imprinted polymers capable of catalysing the Diels-Alder reaction between benzyl 1,3-butadienylcarbamate (1) and N,N-dimethyl acrylamide (2). The polymer systems studied here demonstrated an unusual cross-linker and temperature dependent behaviour, namely that polymer catalysis of the Diels-Alder reaction was lower at elevated temperature, in contrast to the solution reaction. Furthermore, not only was the catalytic activity significantly influenced by the choice of cross-linker, but in a similar fashion also the extent of the temperature effect, indicating a close relationship between catalysis and the observed inhibition. Molecular dynamics simulations of both the polymer systems studied were used to provide insight into the molecular background of transition state stabilisation, and differences in properties of the systems based on different cross-linkers.

    Abstract [en]

    Four different complexes of the cobalt(II) acetate–pyridine–water system were obtained as dominant species by crystallization from a series of dichloromethane and toluene solutions. The complexes were characterized by terms of X-ray crystal structure determination. Factors in solution properties leading to crystallization of certain complexes are discussed. Furthermore, trends in terms of structure and binding energies in a systematic series of mononuclear cobalt(II) complexes were studied using density functional calculations.

    Density functional calculations are employed in the study of the C-C bond formation step of an aldol reaction in presence of a series of metals. Focus was placed on first row d-block metals that have been used in catalysis of direct aldol reactions. The obtained energy profiles are analysed in order to differentiate between factors governing catalysis. Results demonstrate a major influence of d-orbital occupation, and suggest some of the so far less commonly used metals as promising candidates for development of new catalytic systems.

    Abstract [sv]

    Syfte: Syftet med studien var att undersöka om personbilsförare uppfyller de visuskrav som finns för att inneha körkort i Sverige.

    Bakgrund: I Sverige måste alla som ansöker om körkortstillstånd genomgå en visus- och synfältskontroll. Denna kontroll görs vanligen i 16 årsålder, då det är tillåtet att börja övningsköra. Efter denna undersökning finns ingen obligatorisk kontroll av vare sig visus eller synfält om man har behörighet AM, A1, A, B, BE och traktorkort. Detta är något anmärkningsvärt eftersom andelen äldre i Sverige ökar och med stigande ålder blir ögonåkommor och ögonsjukdomar mer frekventa. Även refraktionsstatusen förändras under årens lopp.

    Metod: Genom en visus-screening undersöktes 100 personer med minst behörighet B. Innan visuskontroll fick samtliga deltagare fylla i en enkät om 6 stycken frågor om sig själva, sin syn och sitt körkort.

    Resultat: Samtliga deltagare klarade visusgränsen för B-behörighet. Även de med högre behörighet klarade sin gräns.

    Slutsats: Samtliga deltagare i studien klarade överraskande men positivt kravet vad det gäller visus för körkort i Sverige. Dock krävs en större och mer omfattande studie för att kunna fastställa detta resultat.

    A novel Salmonella serovar was isolated from Peregrine falcon (Falco peregrinus) nestlings in northern Sweden in 2006. Three isolates of the same clone was retrieved from three falcon siblings and characterized as Salmonella enterica sub-species enterica: O-phase 13, 23:-: e, n, z 15 and the H-phase was not present. We propose the geographical name Salmonella enterica, sub-species enterica serovar Pajala to this novel Salmonella.

    Escherichia coli bacteria with extended-spectrum beta-lactamase (ESBL) type CTX-M resistance were isolated from water samples collected close to research stations in Antarctica. The isolates had bla(CTX-M-1) and bla(CTX-M-15) genotypes and sequence types (ST) indicative of a human-associated origin. This is the first record of ESBL-producing enterobacteria from Antarctica.

    Abstract [en]

    A manmade stormwater wetland in Kalmar, SE Sweden, sized 1 ha and receiving water from residential and road areas, was monitored over the first years after inundation with respect to chemistry and biology. Water flow and chemistry was analysed in years 2-4, mainly on a monthly basis, but, in the final year, every second month. This revealed that total nitrogen, according to the Swedish Environmental Quality Criteria (EQC), typically showed moderate or high concentrations, whereas total phosphorous levels were very high or extremely high. Metal (Cd, Cr, Cu, Pb and Zn) concentrations were low or moderate in terms of EQC. Yearly average reduction of nitrogen was 173 kg ha(-1) y(-1), tending to increase over time, and for phosphorous 12.1 kg ha(-1) y(-1), tending to decrease. Vegetation analysis was performed in years 1-4 by noting all species in 27 consecutive zones around the wetland system. This showed that one year after filling with water, the vegetation was already well established with >30 plant species in the entire pond system, and this increased only slightly. After four years, the shoreline vegetation cover had become denser, especially with larger graminoids such as common reed (Phragmites australis) and sea club-rush (Bolboschoenus maritimus), and submersed vegetation almost disappeared. There was a tendency for common species to become more dominant, and for less common species to become rarer. Using sweep net sampling of benthic invertebrates during years 0-2, ca 50 species/higher taxa were observed during the first year, largely because of the appearance of many beetles, especially dytiscids. However, these decreased the following years. Apart from these animals, in the first few months the invertebrate colonisation was dominated by chironomids and corixids, whereas later prominent increases were noticed for the isopod Asellus aquaticus, the snail Physa fontinalis, and the mayfly nymph Cloeon dipterum. The results are discussed in terms of wetland values for biodiversity and nutrient reduction, suggesting that these objectives seem possible to combine in stormwater wetlands.
