    Abstract [en]

    People with severe mental illness and co-occurring substance abuse are an especially vulnerable group in society as health care for those is shared between municipality and county council. There is an associated health need of this target group so the interaction of the social services and the psychiatric care is of great importance. However, there are shortcomings in this interaction with results in that people with dual diagnosis do not always receive adequate care. Diverse knowledge and explanatory models may create problems for the interaction as they include different ways to define, understand, explain and deal with a problem. The purpose of this study was to examine how the interaction between the social service addiction treatment and the psychiatric care is, concerning people with dual diagnosis, focusing on how their respective knowledge and explanatory models affect the interaction. The study was conducted in a county where we interviewed seven people in the social services addiction treatment and the psychiatric care. We used qualitative interviews conducted with the help of a semi-structured variant. The theoretical perspectives that were applied in the study were the theories of; interaction, science- and knowledge-based controversies, and some concepts from the new institutionalism. We concluded that interactions occur in various forms and at a high level of ambition between the social services addiction treatment and the psychiatric care in regards to people with dual diagnosis. We also concluded that the diverse professional knowledge and explanatory models affects the interoperability through the creation of theoretical controversies.

    De nordiska länderna har en låg andel religiöst praktiserande individer i jämförelse med övriga världen. Religionen har liten betydelse för nordbornas dagliga liv. Kyrkobyggnader används vid övergångsriter men står tomma veckans allra flesta dagar. Den ökande andelen människor som av olika anledningar lämnar sina länder och flyttar till detta sekulariserade samhälle möter en begreppsvärld som skiljer sig från den av religionen grundade. De möter ett samhälle som saknar religiösa symboler, vanor och rutiner som är grundläggande för ett samhälle . Vart de än rest på sin väg har de nått olika former av religiöst liv som genomsyrat de länder de passerat, men när de passerat gränsen till de nordiska länderna blev det annorlunda. Hur fyra medelålders kvinnor med ursprung i olika världsdelar anpassar sig till ett samhälle med en främmande religiös begreppsvärld är föremålet för denna studie. De har intervjuats och resultatet talar för en ontologisk otrygghet för immigranter samt att ljuskäglan föll tillbaka på intervjuaren och frågeställningen om vem som är annorlunda. Det som visade sig var att det är skandinaverna som är annorlunda. De upplevs öppna i sinnet, vänliga och ett föredöme vad det gäller rättvisa och jämlikhet men använder inte religionen som rättesnöre i livet. Immigranter upplever en svårighet att förstå hur etik och moral kan upprätthållas utan en gud.

    Abstract [en]

    The purpose of this study was to examine the cooperation between two parts of the Swedish prison and probation service, the prison and the probation office. More specifically its aim was to find the attitudes and the different work models surrounding this cooperation. To find my result I had interviews with six probation officers in two probation offices in south of Sweden. My questions were general and I choose follow-up questions during the interviews. The analysis was based on a cooperation theory. The conclusions of the study were that there are work models that have been brought in by the head office of the organization, that does not work properly and they are not popular by the staff. This keeps the probation officers from having a good cooperation with the staff from the prisons. The probation officers also have some attitudes toward the prison staff that keeps them from having a good cooperation. These attitudes are clearly shown during the prison and probation service’s staff education and somewhat continues to show up during the probation officers every day work. These problems can be explained through the cooperation theory as a lack of perquisite for a good cooperation.

    Title: Double exposed - A qualitative study of professional’s opinion about effort and needs of effort for abused women with substance abuse in Kalmar.

    Women with substance abuse are more likely to be exposed to violence. Despite this, they are having a hard time to get help and support from social service. The purpose of this study is to analyze how professionals express their experience about efforts and needs of efforts for abused women with substance abuse problems.

     

    Questions:

    1. How do professional´s in Kalmar think about the efforts for abused women with substance abuse problems?

    2. How do professional´s in Kalmar think about the needs of efforts for abused women with substance abuse problems?

    3. How could the preventive work be done?

     

    This study is qualitative and was done with four semi-structed interviews with four professionals who work in various social services in Kalmar. The results show that two of these services offer support for victims of violence and two of them offer help for substance abuse problems.  I found that the efforts are good but there is also a considerable lack of effort, especially when it comes to accommodation for these women. There is nowhere for these women to go if there would be in urgent need of help.

    The results were analyzed using theories as normalization processes and labeling theory and on the basis of reality, strategy and the ideal.

    In today's social work there is an ambition to work evidence-based in order to improve quality and competency in business. This study is made in a disability administration which has introduced a common approach towards users for all employees. The approach is called low arousal approach. The purpose of the study is to investigate how a method takes hold and implements within an organization. The empirical data is analyzed by theories of professionalization, idea dissemination and translation. The results demonstrate that the studied administration is influenced by the outside world’s quest for evidence-based work and professionalization in care and welfare sector. It also shows how a method changes when it enters a new context and the consequences the implementation has in the studied administration.

    The purpose of the analysis is to see how welfare officers meet children in distress and how welfare officers converse with children in distress.The analysis has been performed through interviews, and six welfare officers in eastern Sweden have been interviewed. The approach of the study has been phenomenological. The results varied, depending on which of the welfare officers was interviewed, and where this person worked. The results have been analyzed trough the crisis theory of Johan Cullberg and the grief theory by Gurli Fyhr.

    Purpose: The aim of this study was to examine feelings of recovery from mental illness in the perspective of individuals participating in Nymålen; a rural- situated facility focusing on recreation- and work- related interventions. The study took place outside of Oskarshamn, Sweden. The study focused on if, and in that case how, Nymålen contributes to recovery from mental illness. We were also interested in which factors affecting the recovery-process, as expressed by the participants themselves.

    Method: Eight individuals from Nymålen participated in individually performed semi- structured interviews. The collected data was then coded and categorized to enable further analysis by using Qualitative content analysis inspired by Graneheim & Lundman (2003). Latent themes that described the underlying meaning of the material were then extracted, using an analysis inspired by Robert K. Merton’s (1967) thesis on manifest and latent functions.

    Results: The findings of this study suggest that Nymålen in some ways contributes to feelings of recovery from mental illness. Nymålens environment including animals, a feeling of non- demanding chores, perceptive personnel and a positive interaction between the participants were the expressed main factors contributing to recovery from mental illness. Factors interrupting the recovery process were also detected, mainly an unhealthy feeling of competition between the participants, the lack of personnel and feelings of neglect from other psychiatric institutions.

    At the turn of the year 2009/2010 there was a change in the regulations on sickness impact. This led to 328 persons in County of Kronoberg had an expired period of sickness benefit from the Social Insurance Office (Försäkringskassan) when they reached the maximum time for sickness or temporary disability compensation. These individuals were offered introductory programs with the employment services. The purpose of this study is to investigate how the population with expired period of sickness benefit from the Regional Social Insurance Office in the County of Kronoberg (Försäkringskassan Kronoberg) is described in the initial stage, and to explore how individuals who achieved expired period of sickness benefit from the Social Insurance Office (Försäkringskassan) talks about the meaning to get an expired period of sickness benefit regarding economic and social issues. The study was inspired by Grounded theory. The method has consisted of both quantitative and qualitative interviews. In total, six persons participated in the surveys. Narrative method was used in the interview with a person with experience of an expired period of sickness benefit from the Social Insurance Office (Försäkringskassan). The interviews indicate that the authorities have different focus on the concept of rehabilitation. This means that what is to be regarded as rehabilitation and the efforts which may benefit the individual depends on which authority has the power of definition. This study is made before preparing administrators for either statistics or working methods were established by the relevant authorities. Longer experience in the field is still missing about what the change means for both individuals who have experience of the expired period of sickness benefit and professionals. This points out; further research in this area is needed.

    Abstract [en]

    The aim of this study is to describe and analyse the cooperation between the public employment service and the refugee reception in a medium-sized municipality in southern Sweden on basis of the establishment reform. The study examines how the collaborative process is regarded when it comes to implementation of the reform, and when it comes to the directives and resource allocation between the employment services and the refugee reception. The study also focuses on the deficiencies and the benefits that can be discerned with the establishment reform at local level.

    A qualitative data collection method has been used. It is semi-structured interviews with workers from the public employment service and the refugee reception. Information materials and evaluations that various authorities have made have also been investigated.

    The study concludes that it is important that the directives are clarified and that a new local and regional agreement will be established on paper. The study also concludes that because of the strong working line policy and the national workfare regime more responsibility is focused on the individual. This can discourage the establishment and integration process of the newly arrived when the concept of inclusion comes to be regarded as synonymous with employability.

    Det finns personer som går till jobbet varje dag och alltid är vid god hälsa, även när deras arbete är krävande. Detta fenomen kan beskrivas som långtidsfriskhet. Syftet med denna kvalitaitva studie var att undersöka vad socialarbetare upplevde påverkade deras långtidsfriskhet. Vi har gjort intervjuer med åtta socialarbetare angående vilka friskfaktorer som hade störst påverkan på deras hälsa. Det empiriska materialet har sammanställts i tre kategorier, vilka är individen, organisationen och fritiden. Vi har också fört ett resonemang kring sjuknärvarons betydelse för långtidsfriskhet. Materialet har analyserats utifrån förljande teorier: Känsla av sammanhang (KASAM), krav- och kontrollmodellen, organisation och byråkrati, socialt stöd samt coping.

    Abstract:

    The purpose with this study was to explore different peoples preconceived opinions of three psychiatric diagnoses; Attention Deficit Hyperactive Disorder (ADHD), schizophrenia and Borderline Personality Disorder (BPD). These designations are accompanied by a definite social stigma, which could have profound effects on a person diagnosed with either disorder. These designations carry the risk of excluding and labelling individuals and could provoke a feeling of abnormality and potentially segregate them from the Community.

     

    This study is a qualitative study of six individuals, (sample size equals six). The rationale behind the selection was to get a broad representation of different individuals’ opinions. A semi-structured interview method was used and the analysis was made according to Social Cognitive Theory. The aim was to investigate if individuals in different occupations have preconceived opinions and what information and material these beliefs are based on.

     

    The result showed that the preconceptions of these diagnoses, had many common denominators independent of the persons professional subgroup

    The Swedish law says that service within social services shall be of good quality.

    For execution of tasks within social services there must be staff with suitable education and experience. The quality of the activities should systematically and continuous be developed and assured (SFS 2001:453).

    How quality within elderly care is defined depends on whom you ask. The government and the national board of health and welfare have their goals for quality, the unit managers have their goals and the care recipient and the relatives have their goals for quality.

    The quality within the elderly care have been frequently discussed during the autumn 2011, especially after the newspaper Aftonbladet showed that there existed severe shortcomings within the quality work in a company who performs elderly care. This brought my interest to the subject and I decided that my studies were going to take the quality work within elderly care in consideration and especially how the unit managers understand and refer to good quality and which tools they use and have to achieve good quality within elderly care.

    I choose to do a qualitative study where I interviewed eight unit managers. The opinions of the unit managers were analysed with help of the theory of human service organizations and street-level bureaucrats.

    The result shows that the unit managers and the national board of health and welfare have the same goals for good quality within elderly care. The study also shows that educated staff and high staff ratio per care recipient not necessarily means that the quality increases. It also shows that the unit managers see their job as a business, in competition with other competitors, to offer the best possible quality to get as good reputation as possible. While the Heads of Unit maneuver has an impact on quality work in nursing homes.

    Abstract [en]

    Purpose – This paper aims to examine the major factors affecting cumulative summation, to empirically examine the major factors affecting satisfaction and to address the question whether patients in Kazakhstan evaluate healthcare similarly or differently from patients in Egypt and Jordan.

    Design/methodology/approach – A questionnaire, adapted from previous research, was distributed to Kazakhstan inpatients. The questionnaire contained 39 attributes about five newly-developed quality dimensions (5Qs), which were identified to be the most relevant attributes for hospitals. The questionnaire was translated into Russian to increase the response rate and improve data quality. Almost 200 usable questionnaires were returned. Frequency distribution, factor analysis and reliability checks were used to analyze the data.

    Findings – The three biggest concerns for Kazakhstan patients are: infrastructure; atmosphere; and interaction. Hospital staff's concern for patients' needs, parking facilities for visitors, waiting time and food temperature were all common specific attributes, which were perceived as concerns. These were shortcomings in all three countries. Improving health service quality by applying total relationship management and the 5Qs model together with a customer-orientation strategy is recommended.

    Practical implications – Results can be used by hospital staff to reengineer and redesign creatively their quality management processes and help move towards more effective healthcare quality strategies.

    Social implications – Patients in three countries have similar concerns and quality perceptions.

    Originality/value – The paper describes a new instrument and method. The study assures relevance, validity and reliability, while being explicitly change-oriented. The authors argue that patient satisfaction is a cumulative construct, summing satisfaction as five different qualities (5Qs): object; processes; infrastructure; interaction and atmosphere.

    A qualitative study of unaccompanied refugee children and our experiences of the reception in Nybro and Växjö municipality.

    Each year, a large number of unaccompanied young people arrive in Sweden. Immigration Service statistics show that in 2010 the majority of thesewere boys from Afghanistan, Somalia or Iraq. The purpose of this study was to analyze the reception of such young people in two municipalities, Växjö and Nybro, based on the experiences of the personnel involved and of the unaccompanied children themselves and children’s living conditions.

    The method chosen was qualitative interviews, with analysis of results using communication theory and identity theory. We present results that reflect what we considered to be the important themes emerging from our interviews. These themes are:

    the children's background, relationships, children's experiences on the need for support and assistance, health care, children's experiences of living, schooling, the importance of language, cultural distinctions. The result of our study shows that the relationships for unaccompanied children is very important for their future, but cultural clashes makes the relationships difficult. A conclusion is that unaccompanied children are in need of help for some time after they arrived in to the municipality. The results also showes that most young enlisted the help of staff at the homes and that the Swedish language is essential for the unaccompanied children to make them independent. These results are broadly consistent with previous research in this subject

    Abstract [en]

    This is a qualitative study that explores the involvement of the relatives in the special housing based on the perspective of fourteen interviewees working in elderly care, at special housing.

    The purpose of this study is to analyze the nursing staff's view of the involvement of the relatives and the presence of care provision at special housing. The study is to find out what can constitute support or obstacles to the relatives.

    Three questions have been presented as the structural basis upon which the objective finds its conclusion. These questions try to explore and identify the forms of support that can help relatives in their involvement in elderly care. There are opportunities and obstacles for the involvement of the relatives in the care of the elderly as well as expectations of nursing staff on the role of the relatives in the care of the elderly which the following study focuses on.

    The research reflected in this study involves the care staff and their experiences and views on the relatives who have their older live permanently in residential care.

    In conclusion, the study identifies similar reasoning among respondents regarding to key aspects of the relatives at the special housing. They emphasizes that relatives are needed there and they are important resource for both the residents and the staff.

    The results indicate that there may be both opportunities and obstacles for the involvement of the relatives. On one side, factors that prevent relatives from their involvement are lack of communication, poor attitude, different views of relatives and staff about care and conflicts between them. On the other side, there are opportunities that can facilitate relatives' involvement, namely the fact that they need support from the staff. The interviewees pointed out that the relationship between relatives and the elderly is the basis of the relatives' involvement.

    Abstract [en]

    This study aims to investigate the political reformation of the psychiatric care in Sweden, that took place 1995. The main purpose is to illuminate the transformation of subjectivity for mentally ill people after the reform 1995. The group of people which are present in the study are those who former were subjects of care in psychiatric hospitals, but with regarding to the reform have moved out from the institutions and instead become clients for community care. This new group of mentally ill people became, in connection with the reform, subjects for a new concept psykiskt funktionshinder. This new label and concept, that were attached to the present group, is the main concept for the investigation in the present study. The method, that was used in the study, develops a textual based analysis of the official documents, that were produced in connection with the political decision to reform the care of mentally ill people. With a theoretical conceptuality taken from Michel Foucault, with concepts such as discourse, power, and subjectivity, are the documents analysed in order to illuminate how the new subjectivity, under the concept of psykiskt funktionshinder arise. The results shows how concepts as time and space are intertwined with strategies of power, that simultaneously function as conditions of possibility for this new subjectivity to exist. The main conclusion, in the study, is that the concept psykiskt funktionshinder must be understood with regard to an analysis of power relations, and that the power, which are latent in the present discourse, is a decisive condition for both clients and social workers to become subjects.
