    Abstract [en]

    Background. Regional variations in mortality and morbidity have been shown in Europe and USA. Longitudinal studies have found increased mortality, dissimilarities in mortality pattern, and differences in utilization of healthcare between foreign- and native-born Swedes. No study has been found comparing mortality among foreign-born and native-born Swedes in relation to catchment areas/counties. Methods. The aim was to describe and compare mortality among foreign-born persons and native Swedes during 1970–1999 in 24 counties in Sweden. Data from the Statistics Sweden and the National Board of Health and Welfare was used, and the database consisted of 723,948 persons, 361,974 foreign-born living in Sweden in 1970 and aged 16 years and above and 361,974 matched Swedish controls. Results. Latest county of residence independently explained higher mortality among foreign-born persons in all but four counties; OR varied from 1.01 to 1.29. Counties with a more rural structure showed the highest differences between foreign-born persons and native controls. Foreign-born persons had a lower mean age (1.0–4.3 years) at time of death. Conclusion. County of residence influences mortality; higher mortality is indicated among migrants than native Swedes in counties with a more rural structure. Further studies are needed to explore possible explanations.

    Abstract [en]

    Aim: to analyze the pattern of mortality in deceased foreign-born persons living in Sweden during the years 1970-1999 in relation to distance mobility.

    MEthods: Data from Statistics Sweden and the National Board of Health and Welfare was used, and the study population consisted of 281,412 foreign-born persons aged 16 years and over who were registered as living in Sweden in 1970.

    Results: Distance mobility did not have a negative effect on health. Total mortality was lower (OR 0.71; 95% CI 0.69-0.73) in foreign-born persons in Sweden who had changed their county of residence during the period 1970-1990. Higher death rates were observed, after adjustment for age, in three ICD diagnosis groups "Injury and poisoning", "External causes of injury and poisoning", and "Diseases of the digestive system" among persons who had changed county of residence.

    Abstract [en]

    In previous longitudinal studies of mortality and morbidity among foreign-born and native-born Swedes, increased mortality and dissimilarities in mortality pattern were found. The aim of this study is to describe, compare, and analyse the utilization of in-hospital care among deceased foreign- and Swedish-born persons during the years 1987–1999 with focus on four diagnostic categories. The study population consisted of 361,974 foreign-born persons aged 16 years and upward who were registered as living in Sweden in 1970, together with 361,974 matched Swedish controls for each person. Data from Statistics Sweden (SCB) and the National Board of Health and Welfare Centre for Epidemiology, covering the period 1970–1999, was used. Persons were selected if they were admitted to hospital during 1987–1999 and the cause of death was in one of four ICD groups. The results indicate a tendency towards less health care utilization among migrants, especially men, as regards Symptoms, signs, and ill-defined conditions and Injury and poisoning. Further studies are needed to explore the possible explanations and the pattern of other diseases to see whether migrants, and especially migrant men, are a risk group with less utilization of health care.

    Abstract [en]

    A limited number of studies have been found on stroke mortality in migrants showing higher mortality for some groups. Influence of time of residence has been studied by one research group. An earlier study showed a significantly higher number of deaths in Diseases of the circulatory system in Finnish migrants compared with native Swedes. To test the hypothesis of a higher mortality in and a decrease in mortality over time in stroke among Finnish migrants in Sweden. The study was based on National Population data, the study population included 321,407 Swedish and 307,174 foreign born persons living in Sweden 1987-1999. Mean age was lower at time for death for Finnish migrants than native Swedes, men 5.1 years difference and women 2.3 years. The difference decreased over time. The risk of death by stroke was higher for migrants with short time of residence than with long time (<= 10 years, OR 1.61-1.36 vs >= 11 year, OR 1.18). Migrants with short time of residence died 9.8-5.3 years earlier than native Swedes. The hypothesis was confirmed and an indication of adjustment to life in the new country was found. International studies show similar results for other migrant groups but further studies are needed to verify if the same pattern can be found in other migrants groups in Sweden and to generalise the findings.

    Abstract [en]

    Cultural background and environmental factors such as UV-radiation and lifestyle during childhood and adolescence may influence the risk of a hip fracture event later in life. Differences in prevalence might occur between the indigenous population and those who have migrated to a country.

    METHODS: The study was based on national population data. The study population consisted of 321,407 Swedish-born and 307,174 foreign-born persons living in Sweden during the period 1987-1999.

    RESULTS: Foreign-born persons had reduced risk of hip fracture, with odds ratios (ORs) of 0.47-0.77 for men and 0.42-0.88 for women respectively. Foreign-born women had the hip fracture event at a higher age on average, but a longer time spent in Sweden was associated with a small but significant increase in risk.

    CONCLUSIONS: There was a reduced risk of hip fracture in all foreign-born individuals, and that the hip fracture event generally happened at a higher age in foreign-born women. Migration must therefore be considered in relation to the prevalence and risk of hip fracture. Migration can therefore have a positive effect on one aspect of the health of a population, and can influence and lower the total cost of healthcare due to reduced risk and prevalence of hip fracture.

    Abstract [en]

    The overall aim with the study was to describe the knowledge building that was developed in a group of students who were trained nurses enrolled in a Swedish higher education programme. With a point of departure in an interactive research approach, the authors applied the dialogue seminar as method and pedagogical model. When the students in close to practice talks, shared their thoughts about how gender and gender equality could be experienced and understood, the dialogue seminar became useable. At the analysis of the stories which the students shared at the dialogue seminars, it became clear that most of the students had an ambivalent approach to gender equality. When they reflected on gender relations, they brought out the hierarchical and separate gender relations which they experienced as existing within healthcare. Furthermore, their professional lives seemed to be embedded in gender-related practices. An important conclusion is that the students’ reflections oscillated between critical reflection on conditions and critical reflection on processes within their own practice in healthcare. Another finding was that the students’ reflections in dialogue form became important in a learning process, not least in the light of that gender relations and gender equality only to a limited degree had been included in their nurse’s education at the basic level.

    Abstract [en]

    Gender equality is a cornerstone of Swedish society. It means that women and men have the same opportunities, rights and obligations in all areas of life and can be seen as part of the social dimension of sustainable development. The definition implies that women and men can work and support themselves, combine work and care of children on the same terms and that neither sex in a relationship need to worry about being subjected to abuse or violence. In Sweden equal opportunities has been on the agenda for the past decade and beyond. Despite the Equal Opportunities Act (1991:433), people still experience disadvantages, especially in the labour market. Although Sweden has come a long way in making sure that women and men are treated equally in the workplace there are few countries with so obvious gender based occupational segregation. Within Swedish health- and medical care for example 81 per cent of the employees are women. The majority of those work in a hierarchical structure where the lower and intermediate levels primarily are comprised of women. With this point of departure we have studied the construction of gender equality in every day meetings between women and men working in health- and medical care. By studying the perspective of equality and by adding gender theories we have been able to focus on the domination process.

    The aim of the presentation is to discuss whether or not a methodological approach can promote a process of making visible gender equality within the health and medical care.

    Although Sweden has come a long way in making sure that women and men are treated equally in the workplace there are few countries with so obvious gender based occupational segregation. Within Swedish health- and medical care for example 81 per cent of the employees are women. The majority of these work in a hierarchical structure where the lower and intermediate levels primarily are comprised of women. With this point of departure we have studied the construction of gender equality in every day meetings between women and men working in health- and medical care.

    Aim

    The overarching purpose of the study was to describe and problematize the perspective of gender equality within the health- and medical care. Two main questions were posed:

    • How is the aspect of gender equality expressed in work related meetings between women and men working in health care?
    • Which opportunities and hindrances exist for creating sustainable gender equality within the health- and medical care?

    Method

    The empirical material comprised 23 women and men studying health care or public health at Blekinge Institute of Technology. The informants were also working as professional nurses. The study was carried through in two steps. Firstly four introductory lectures were conducted. Two of them were about qualitative methods with a particular emphasis on focus groups as an interview technique. The other two focused on current gender equality research. The informants also deepened their knowledge by reading articles about gender equality. In the second step qualitative interviews in focus groups were conducted. Before these occasions the interviewees had prepared themselves by reading a research report which made it possible to relate the text to personal experiences from working life.

    Result

    The empirical material showed that the knowledge about gender equality differed within the studied group. Most informants considered the problem area of vital importance and stressed the necessity of continual discussions in the workplace. However some of the interviewed had never thought of the issue as they regarded gender equality being a question without any interest. All our informants were aware of the plan of action for equality which is required at the workplace but only few of them had read it or worked actively with the implementation. When the interviewees reflected upon the hierarchical structure in their workplaces a common view was that an obvious difference between women’s and men’s opportunities to influence and exercise domination could be seen.

    Abstract [en]

    With the departure in emotion sociology and caring science the aim of this article is to elucidate the emotion work as experienced by a group of nurses who tried out a customised form of a person-centred care model. Ten semi-structured interviews with following go-along were conducted. The most important empirical finding is the identifying of emotional caring as a specific part of the nurses' emotion management, comprising such knowledge that is specific to the competence of nurses. Emotional caring thus forms part of caring science, affected by the organisational structure, with bearing on the nurses' room for caring actions, that is, how, when and in what way emotional caring can be carried out. Moreover, in practice, person-centredness was compatible with the commonly shared values in caring science. The empirical material also shows that emotion management can be tied to profession, positions, status and power. The article contributes with an understanding of how working in a person-centred care model made it possible for the nurses to come closer to the patient's life world. Qualitative descriptions of the emotion management of a group of nurses in a certain caring context constitute another contribution, something that has not been investigated before.

    Syftet med projektet är att mäta hälsoeffekter av total höftledsplastik med hjälp av hälsoindex och höftindex, att göra en beräkning av kostnaderna för total höftledsplastik inom hälso- och sjukvårdssystemets olika delar, samt att analysera skillnader i resursförbrukning vid Karlskrona och Karlshamns lasarett. Hälsoindexen Rosser, Sickness Impact Profile (SIP) och Euroqol har använts och visar förbättrat hälsoindex 6 månader efter operationen. Höftindexen Charnley, Andersson-Möller/Nielsen och Harris har också testats. Avgörande skillnader mellan höft- och hälsoindex påvisas ej. Kostnaden för ett ingrepp är ca 60 000 kr. Variationer i kostnader mellan olika kliniker kan förklaras med olika behandlingsrutiner.

    Abstract [en]

    This article aims to describe the phenomenon of learning at a university-based nursing student–run health clinic, as experienced by student nurses and lecturers. The study is based on a reflective lifeworld research approach founded on continental philosophy. Eight group interviews were conducted with 38 student nurses and 5 lecturers. The data were explored and analysed for meaning. The results show that learning is supported by a permissive learning environment that builds on both individual and common learning as well as equal relationships within the student group, in relation to the visitors at the health clinic and, to a certain extent, in relation to the lecturers. The most significant finding is that reflective, development-oriented learning takes place when the students, supported by each other and their lecturers, reflect on how to relate to problems and situations. A situation-based learning approach is thus shown to create the prerequisites for lecturers being nearby, reflective dialogue partners but also supervisors in situations where the students ask for support and guidance.

    Abstract [en]

    Background: All healthcare in Sweden is voluntary and must be given in informed consent. A dilemma arises when people no longer themselves can express their will and then not express his desire for health care. Advance Directive, which occur in many other countries, is a way for patients to communicate their desire for future care. In Sweden, advance directives are not legally binding and it is unclear if and in what incidence it exists. Nurses in home health care encounter many patients who no longer is expected to make their own decisions, which should give them the experience of the problem of patient autonomy. Purpose: The aim of the study was to investigate the nurses' experiences of and attitudes towards advance directives. Method: A quantitative descriptive design was used based on a questionnaire. The data was analyzed in SPSS version 21. Results: This study describes nurses' experiences and attitudes in a county in southern Sweden. The results shows that nurses have met patients who have prepared advance directives, and that the directives have not always been followed. At the same time, there was some uncertainty about Swedish policy on advance directives and if health care professionals must follow an advance directive. It is noted that the patient's right to self-determination is central and that attitudes to advance directives are positive. Conclusion: There is among some patients today need to write advance directives and if health care professionals can meet patients who completed advance directives, their knowledge of the directives must be improved. The nurses in home health care are positive about advance directives as a way to strengthen the patient's right to self-determination, and the problem area should continue to be studied, focusing on different contexts, other professionals and patients' views.

    Background: Obesity has become one of our times most endemic disease on a global scale and changes to lifestyle is the most cost-effective way to treat patients, when the cost for healthcare related treatment is staggeringly high for obesity and sequela diseases NAFLD, diabetes typ 2, dyslipidaemia and metabolic syndrome.The problem with this remedy is that it requires work and dedication. But changes require hard work, and in this patient group- low compliance, weight gain after treatment, dropping out of programs and small desire to change are the most common problems. Motivational studies report that readiness in obese patients is low and the best way to help patients to move forward is by motivational conversations. The obesity sequela disease NAFLD is an asymptomatic disease it displays no symptoms until very late stages. Therefore it’s a problem to get patients make the patient understand his illness and the seriousness of it.

    Aim: This literature work was aimed at investigating compliance in lifestyle changes in obese subject and to see if close contact with healthcare staff affected the achieved results.

    Method: In this literature study, the databases Pubmed, Science Direct, Medline and Sportdiscus were used to find information. Article inclusion criteria were that the articles were not older than 10 years and were in English.

    Result: Frequent and regular contacts between participants and professional staff provided good results both with regard to weight loss, biochemical response, and the participants' willingness to change. Also it shows that return visits at least every three months will improve weight loss if the participant is motivated to implement a change to lifestyle.

    Conclusion: Overall, this literature study shows the difficulties with lifestyle changes in people with obesity and sequela NAFLD. Close contacts of the patients with healthcare staff has proven to have a positive impact on treatment compliance, but there are other lifestyle difficulties in these patient groups which hamper compliance.

    Bakgrund: Varje år insjuknar ca 300 barn i cancer och cancern ger upphov till tjugo procent av alla dödsfall bland barn. Orsaken till cancer hos barn och ungdomar är okänd. För föräldrarna blir det ett hot om att mista sitt barn när de blir varse informationen. Föräldrarna upplever en genomgripande förändring i sina liv, med förlust av kontroll och allt blir kaos. Syfte: Att belysa föräldrars upplevelser och copingstrategier i samband med att deras barn har drabbats av cancer. Metod: Systematisk litteraturstudie med elva inkluderade artiklar. Resultat: Resultatet bildade tre teman; känslor, kommunikation och copingstrategier. Föräldrarna beskrev ett flertal olika känslor i samband med barnets sjukdom, såsom oro och skuld. Kommunikation och information från sjukvårdspersonal var av stor betydelse för föräldrarnas välbefinnande. För att hantera situationen använde sig föräldrarna av olika strategier, såsom att söka stöd och att söka existentiellt svar. Slutsats: Det är viktigt för sjukvårdspersonal att ha kunskap om olika reaktionssätt hos föräldrar till barn som har drabbats av cancer. Detta för att lättare kunna behandla föräldrarna med respekt oavsett hur de valt att hantera situationen.

    Abstract [en]

    Nursing homes are often places where older persons come to die. Despite this, death and dying are seldom articulated or talked about. The aim of this study was to explore assistant nurses' experiences of conversations about death and dying with nursing home residents. This study is part of an implementation project through a knowledge-based educational intervention based on palliative care principles. An ethnographic study design was applied in seven nursing homes, where eight assistant nurses were interviewed and followed in their daily assignments through participant observations. The assistant nurses stated that they had the knowledge and tools to conduct such conversations, even though they lacked the time and felt that emotional strain could be a hinder for conversations about death and dying. The assistant nurses used the strategies of distracting, comforting, and disregarding either when they perceived that residents' reflections on death and dying were part of their illness and disease or when there was a lack of alignment between the residents' contemplations and the concept of dying well. They indicated that ambivalence and ambiguity toward conversations about death and dying should be taken into consideration in future implementations of knowledge-based palliative care that take place in nursing homes after this project is finalized.

    Bakgrund: Andelen barn med medfött hjärtfel som når vuxen ålder har ökat stort tack vare förbättrad vård, diagnostik och behandling. Genom denna utveckling har en ny patientkategori tillkommit i den kardiologiska vuxenvården om vilken de sjuksköterskor som arbetar inom densamma, inte innehar så stor kunskap. Syftet med denna studie var att belysa hur vuxna och ungdomar upplever att leva med ett medfött hjärtfel. Metod: För att utforska detta gjordes en systematisk litteraturstudie vilken resulterade i en innehållsanalys av nio kvalitativa artiklar. Resultat: Personer med medfött hjärtfel upplever att de är annorlunda gentemot sina jämnåriga, detta tar sig uttryck i fysiska skillnader, begränsningar, kroppsuppfattning och känslomässiga uttryck såsom utanförskap, skuld och rädsla. Sättet som de hanterar detta på påverkas av personligheten, upplevelsen av vårdmöten och omgivningen. Slutsats: Denna studie handlar om en växande patientgrupp och sjukvården bör ta till vara deras kunskap om hur det är att leva med ett medfött hjärtfel. Genom att ta del av patienternas upplevelse av sin sjukdom kan sjuksköterskan utöka sin kunskap och därigenom bättre möta denna patientgrupp och på så sätt bidra till att utveckla vården för vuxna patienter med medfött hjärtfel.

    Denna studie är en kvantitativ enkätstudie riktad till föräldrar som deltagit i föräldrautbildningar inom svensk fotboll i Stockholm län. Studien syftar till att öka kunskapen kring fotbollsföräldrars inställning och upplevelse av deras deltagande i föräldrautbildningar som Svenska fotbollförbundet bedriver i Stockholms län. Studien tar sin utgångspunkt i den debatt kring föräldraengagemang inom svensk idrott som tagits upp i media under de senaste åren.

     

    Respondenterna i studien är föräldrar som deltagit i föräldrautbildningar inom fotbollen i Stockholms län där de fått beskriva sin egen inställning och upplevelse av dessa utbildningar.

     

    I studien har ett teoretiskt perspektiv kring kognitivt inlärande i grupputbildningar använts i en diskussion samt tidigare forskning inom samma och närliggande ämnen.

     

    Resultatet i studien bidrar med en ökad förståelse kring hur föräldrar upplever de utbildningar som annordnats samt deras inställning. Utifrån detta har studien kunnat visa på vad i utbildningarna som varit bidragande till dessa inställningar och upplevelser samt vad i utbildningen som de upplevt bidragit till en ökad förståelse kring vikten av att vara en god fotbollsförälder för barnens skull.

    Bakgrund: Den psykiska ohälsan ökar både i Sverige och internationellt och ses idag som ett av de stora folkhälsoproblemen. Patienter med psykisk ohälsa kan även få behov av att uppsöka den somatiska vården. Tidigare forskning har visat att både patienter och sjuksköterskor kan uppleva svårigheter och problem då patienter med psykisk ohälsa vårdas inom somatiken.

    Syfte: Syftet med studien var att belysa sjuksköterskors upplevelser av att bemöta och vårda patienter med psykisk ohälsa på somatiska vårdavdelningar i södra Sverige.

    Metod: En kvalitativ undersökning utfördes med hjälp av semistruturerade frågeformulär. Den insamlade datan analyserades med hjälp av kvantitativa analysmetoder vad gällde bakgrundsfrågorna, medan huvudfrågorna analyserades med en kvalitativ manifest innehållsanalys.

    Resultat: Sjuksköterskorna uppgav varierande upplevelser vid bemötandet och vårdandet av patienter med psykisk ohälsa som sammanställdes i följande kategorier; Uppfattningar och attityder om psykisk ohälsa, Upplevelser av att vårda patienter med psykisk ohälsa, Hinder för en god vård samt Faktorer som kan underlätta bemötandet och vårdandet.

    Slutsats: Sjuksköterskorna upplevde flera svårigheter som brist på kunskap och tid, vilket kunde försvåra säkerställandet av en god vård till patienterna och orsaka dem ett vårdlidande. Därför är det av vikt att adekvata åtgärder vidtas för att förbättra för både sjuksköterskor och patienter.

    Maria-mottagningarna i Stockholm, Göteborg och Malmö - Ungdomar i öppenvård år 20132014Report (Other academic)

    Trestad2 är en nationell satsning där de tre städerna Stockholm, Göteborg och Malmö samarbetar för att minska användandet av cannabis bland ungdomar. Inom ramen för projektet har det skapats en modell för att identifiera och följa trender i de tre storstäderna vad gäller droganvändning och psykosocial situation för ungdomar som påbörjar behandling för missbruksproblem. Framtagandet av relevanta indikatorer har skett i nära samarbete mellan praktiker och forskare utifrån en sammanvägning av forskningsmässiga, metodologiska och kliniskt betydelsefulla utgångspunkter. Underlaget utgörs av uppgifter om 788 ungdomar som inledde öppenvårdsbehandling vid någon av städernas Maria-mottagningar under år 2013 och har inhämtats med intervjumetoden UngDOK.

    Sammanställningen av indikatorer visar att flickor utgör cirka en fjärdedel av det totala antalet ungdomar som påbörjar öppenvård för problem med alkohol eller narkotika. Ungdomarnas medianålder när vårdkontakten inleds är 17 år och mer än hälften av ungdomarna har kommit i kontakt med Maria-mottagningarna via eget eller nätverkets initiativ. Nästan två tredjedelar av ungdomarna uppger att de har eller har haft omfattande problem under sin skolgång och cirka en tiondel av ungdomarna varken studerar, arbetar eller praktiserar. Drygt tre fjärdedelar av ungdomarna anger cannabis som sin primära drog, men nästan hälften av dem har även en riskfylld alkoholkonsumtion. Den genomsnittliga debutåldern för den primära drogen är 15 år. Ungefär en fjärdedel av ungdomarna har tidigare fått vård och behandling för alkohol- eller narkotikaproblem. Förekomsten av besvärliga uppväxtvillkor är hög hos ungdomarna och över hälften av dem har erfarenheter av att ha växt upp med våld, psykiska problem och/eller missbruk i familjen. Drygt en tredjedel av samtliga ungdomar har eller har haft kontakt med den psykiatriska vården.

    Vissa skillnader mellan de tre städernas Maria-mottagningar kan identifieras. Ungdomarna som påbörjar kontakt med mottagningen i Stockholm verkar ha problem av något lindrigare art jämfört med Göteborg och Malmö och utgör mindre andelar både när det gäller komplicerande bakgrundsfaktorer och nuvarande problembeteenden. Det finns även mindre skillnader i åldersstrukturen i de tre städerna, där Stockholm har fler ungdomar under 18 år medan både Göteborg och Malmö har fler som är över 18 år. Det finns även vissa könsskillnader genom att flickorna i samtliga tre städer tycks ha en tyngre problematik i jämförelse med pojkarna. Flickorna saknar i högre grad sysselsättning, anger oftare problem i skolan, har haft svårare uppväxtvillkor och har i större utsträckning haft kontakt med psykiatrin i jämförelse med pojkarna.

    Att sammanställa och rapportera deskriptiva indikatorer om individer som påbörjar behandling för alkohol- och narkotikaproblem kan ha flera fördelar. En sådan är att informationen kan ligga till grund för en ökad förståelse av ungdomars alkohol- och narkotikaanvändning och dess omfattning. Även om rapportens uppgifter inte är heltäckande, beskriver den en större population av svenska ungdomar med en problematisk användning av alkohol och droger. Föreliggande rapport är således en första ansats till att ge en sammanfattande bild av de ungdomar som påbörjar behandling vid Maria-mottagningarnas öppenvård i Stockholm, Göteborg och Malmö.

    Abstract [en]

    This thesis focuses on the relationship between technology and caring in technologically intense environments. The overall aim was to uncover the meaning of care in those environments as experienced by patients and caregivers. Moreover, the study aimed at finding a deeper understanding for the almost total dominance of technology in care in intensive care.

    The thesis includes three empirical studies and one theoretical, philosophical study. The research was guided by a phenomenological and lifeworld theoretical approach. Research data consist of quantitative parameters and qualitative interviews with caregivers and patients. Data was analysed and synthesised with aim of seeking meaning through openness, sensitivity and a reflective attitude. The goal was to reach the general structure of the phenomenon and its meaning constituents.

    The result shows that an intensive care unit is a cognitive and emotionally complex environment where caregivers are juggling a precarious handful of cards. Despite being constantly monitored and observed, intensive care patients express that they feel invisible. The patient and the apparatus easily meld into a unit, one item to be regulated and read. From the patients’ perspective, caregivers demonstrate keen vigilance over technological devices and measured parameters, but pay scant attention to their stories and experiences. Technology, with its exciting captive lure and challenging character, seduces the caregivers and lulls them into a fictive sense of security and safety. Technical tasks take precedence or have more urgency than caring behaviour.

    A malaise settles on caregivers as they strive for garnering the security that technology promises. Yet simultaneously, insecurity creeps in as they read the patient’s biological data. Technical tasks take precedence over and seemingly are more urgent than showing care. Listening, inspiring trust, and promoting confidence no longer have high priority. Trying to communicate ‘through’ technology is so complex, that it is a difficult challenge to keep in perspective what or who is the focus; ‘seeing’ or caring. Technology should be like a catalyst; do its ‘thing’ and withdraw ‘unnoticed’.

    This thesis has contributed in gaining deeper knowledge about care in technologically intense environments and the impact of technology. The main contribution is that caregivers need to be aware that the roar of technology silences the subtle attempts of the critically ill or injured person to give voice to his or her needs. In conclusion, the challenges for caregivers are to distinguish when to heighten the importance of the objective and measurable dimensions provided by technology and when to reduce their importance. In order to magnify the patients’ lived experiences. It is a question of balancing state-of-the-art technology with integrative and comprehensive care, of harmonizing the demands of subjectivity with objective signs.

    At stake in this study is the patient's credibility. The Cartesian philosophical standpoint, which holds sway in western thinking, questions with scepticism whether the reported symptoms are “real.” Do they reside in the body, or are they mentally concocted. However, from the caring perspective any symptom must be both listened and attended to in its own right, not just scrutinized as evidence for an accurate diagnosis.

    In cognitively and emotionally complex high-tech units caregivers are juggling a precarious handful of cards. Technical tasks take precedence or have more urgency than caring behaviour. Assuming an irremediable tension between object–subject and care–cure in nursing is futile dualism. By addressing the essence of technology—the non-neutral and highly visual technology—this paper aims to find, from a philosophical point of view, a more comprehensive understanding for the dominance visualism and technification within intensive care.

    Screens give us access to vital signs. Screens record numbers and lines that relate to a graph and afford superfine spiked “readings.” However, the most relevant vital signs may be missing.

    Bakgrund: Cirka 600 kvinnor i Sverige drabbas av hjärntumör varje år. Symtom och behandling ser olika ut beroende på hjärntumörens lokalisation och utbredning. Livet förändras och framtiden blir oviss i samband med diagnostisering, sjuksköterskan har en betydande roll under sjukdomstiden. Familjen blir också drabbad, vardagen är inte längre sig lik och prioriteringarna förändras.

    Syfte: Syftet är att belysa kvinnors upplevelser av att leva med hjärntumör.

    Metod: Studien är en kvalitativ litteraturstudie som baseras på fem självbiografier skrivna av kvinnor. Urvalet begränsas utifrån beskrivna inklusions- och exklusionskriterier. En manifest innehållsanalys har används vid analys av data.

    Resultat: Kvinnornas upplevelser av att leva med hjärntumör beskrivs genom tre kategorier och tio underkategorier. Det framkommer att kvinnor kan känna både välbefinnande och lidande under sjukdomstiden. Upplevelserna beskrivs på olika sätt hos kvinnorna men rädsla och att finna styrka trots sjukdom är framträdande. En annan faktor som upplevs av betydelse är stöd från omgivningen, i form av familj, vänner och vårdpersonal.

    Slutsats: I studiens resultat framkom det att kvinnor som lever med hjärntumör uppnådde välbefinnande när de accepterade sjukdomen. Omprioritering och att se livet på ett nytt sätt blev tydligt. Kvinnorna upplevde lidande genom en förändrad identitet, rädsla, ensamhet, hopplöshet och förtvivlan över barnen. Sjuksköterskan måste därför se varje kvinna som unik för att skapa bästa möjliga vård.

    Bakgrund: Antal personer som behandlas med dialys i Sverige har ökat under de senaste 20 åren. Att vara njursjuk och behandlas med dialys berör inte bara den sjuka utan påverkar även hela familjen. Den teoretiska referensramen som används är systemteorin som är en del av Calgarymodellen. Denna ligger till grund för den familjefokuserade omvårdnaden som hjälper familjen att belysa problem och finna lösningar.

    Syftet med studien var att belysa familjens upplevelser och hur familjen hanterar sin situation när en familjemedlem behandlas med dialys.

    Metod: En systematisk litteraturstudie som inkluderarade nio artiklar. En kvalitativ innehållsanalys användes för att analysera resultatet.

    Resultat: Huvudfynden delades upp i familjens upplevelser och hur familjen hanterar sin situation. Resultatet visade på att familjen påverkades mycket både psykiskt, fysiskt och socialt samt att de skapade egna strategier för hur de skulle klara av sin situation. 

    Slutsats:. När dialysbehandlingen startar finns det ingen uppföljande verksamhet dit anhöriga kan vända sig. Mycket av forskningen inom dialysvården är inriktad på patienten. Resultatet är betydelsefullt för personalen som arbetar med dialysvård. Det ger en ökad förståelse i mötet med både patienten och dennes familj, då dessa påverkar varandra.

    Bakgrund Många människor har på senare år tvingats fly från sina hemländer. Detta har resulterat i att vårdpersonalen vårdar allt fler människor med olika kulturella bakgrunder. Vårdpersonal möter allt oftare flyktingar och asylsökande som bär på traumatiska upplevelser och skiftande hälsotillstånd. Att vårda människor från andra kulturer kan vara utmanande för vårdpersonalen då de ska kunna ge en god kvalitativ vård på lika villkor till alla människor. Syfte Att belysa vårdpersonalens erfarenheter av att vårda vuxna flyktingar och asylsökande. Metod Utformad som en litteraturstudie. Sammanlagt valdes 8 kvalitativa artiklar ut från databaserna Cinahl och PsycINFO till kvalitetsgranskning. Resultatet diskuterades i förhållande till den valda teorin. Resultat Delas in i 2 kategorier och 7 subkategorier. Första kategorin är Vårdpersonalens erfarenheter av att kommunicera där språkbarriärer och tolk som hjälpmedel blev subkategorier. Den andra kategorin Vårdpersonalens erfarenheter av att vårda innefattar ohälsa kopplat till flyktingar, kulturella skillnader, grundläggande aspekter i vårdandet, utmaningar för vårdpersonal samt betydelsefullt och utvecklande arbete som subkategorier. Vårdpersonal upplevde att det var svårt att kommunicera. Att använda tolk var problematiskt. Krig, våld, olyckor och ohälsosamma levnadsförhållanden bidrog till en mängd olika hälsobesvär. Kulturella skillnader försvårade omvårdnaden. Familjefokuserad omvårdnad, en stödjande vårdrelation med förtroende och möjlighet till samtal var viktiga aspekter i omvårdnaden. Resultatet diskuterades utifrån Madeleine Leiningers teori om vård av kulturell mångfald och universalitet. Slutsats Det behövs en större kulturell medvetenhet bland vårdpersonal för att kunna bemöta och ge en god kvalitativ vård till flyktingar och asylsökande.

    Bakgrund: Människor korsar geografiska gränser av olika anledningar. Sjukvården möter patienter från hela världen. Kulturella uttryck annorlunda än de sjuksköterskan har som norm har tendensen att skapa problem i det omvårdande mötet. Den personcentrerade omvårdnaden riskerar att påverkas negativt när kulturella svårigheter dem emellan måste hanteras.

    Syfte: Att belysa sjuksköterskors erfarenheter vid omvårdnad av patienter födda i ett annat land.

    Metod: En systematisk litteraturstudie enligt Forsberg och Wengström (2008) genomfördes med sökning av artiklar i Cinahl, PubMed och PsycINFO.

    Resultatet: Fyra kategorier. Kulturella skillnader, Brist på kunskap, Känslor och reaktioner och  Språksvårigheter. Annorlunda matvanor, religionsutövande, närvaro av släkt och relationen till döden exemplifierar hur de kulturella skillnaderna påverkade sjuksköterskorna. En känsla av osäkerhet handlade om situationer som sjuksköterskorna beskrev i mötet med patienten från ett annat land när kulturella skillnader fick dem att känna sig obekväma, eller frustrerade. Sjuksköterskorna uttryckte en brist på kunskap inom området och uttryckte önskemål om kulturspecifik utbildning för att höja sin kulturella kompetens. Språksvårigheter var problematiska, främst när anhöriga tolkade för patienten. Ovissheten om översättningens korrekthet och relationen mellan den som översätter och patienten ses som en risk för patientsäkerheten.

    Slutsats: Sjuksköterskan behöver utbildning i kulturell kompetens och att arbetsgivaren behöver bereda utrymme för utbildning.

    Bakgrund Inom palliativ vård möter sjuksköterskor huvudsakligen existentiella frågor relaterade till döden. Dock är det inte alltid sjuksköterskor vill möta dessa frågor, vilket kan skapa lidande för patienten. Vald teoretisk referensram utgörs av Travelbee’s teori om mellanmänskliga relationer och Watson’s teori om att skapa en hjälpande och tröstande relation. Syfte Att belysa sjuksköterskans erfarenheter av existentiella frågor i möten med patienter som vårdas  i den palliativa vårdens sena fas. Metod Metoden utgörs av en systematisk litteraturstudie där artiklar hittades i databaserna Cinahl, PubMed och PsycINFO. 10 artiklar valdes ut för analys efter kvalitetsgranskning och diskuterades mot vald teoretisk referensram. Resultat Resultatet beskrivs utifrån tre teman med de nio subteman som identifierats. Resultatets tre teman är: att uppmärksamma existentiella frågor, att våga möta existentiella frågor och att skapa förutsättningar för att möta existentiella frågor. Slutsats Vår systematiska litteraturstudie belyser vikten av att som sjuksköterska kunna uppmärksamma, våga möta och kunna skapa förutsättningar för att möta existentiella frågor. Dock finns det brister inom sjuksköterskans kompetensområde och organisation. Därmed kan vi dra slutsatsen att det finns behov av utbildning avseende existentiell vård inom sjuksköterskans kompetensområde och tydligare riktlinjer från organisationen för att förbättra sjuksköterskors förmåga att möta existentiella frågor.

    Att vara ute i naturen medför inte bara rekreation utan kan kanske även bidra till en ökad miljömedvetenhet. Dock visar forskning skilda resultat i frågan. En ökad miljömedvetenhet är är viktigt för att främja en hållbar utveckling, därför är det intressant att undersöka hur ungdomar ställer sig till miljöfrågor. Syftet med denna studie var att undersöka om det finns ett samband mellan olika friluftsaktiviteter och miljömedvetenhet hos ungdomar. I studien inkluderades enkäter (n=433) från högstadie- och gymnasieungdomar i en medelstor kommun i södra Sverige. Data samlades in och kodades om till siffror för att sedan analyseras med Spearman rank colleration och Mann - Whitney U - test. Resultatet visade att det fanns skillnader mellan årskurser men att ungdomar har en låg miljömedvetenhet och att friluftsaktiviteter inte har något eller svagt samband med miljömedvetenhet.

    Abstract [en]

    This study investigated the association between spinal alignment, mobility of the hips and the thoracic spine and low back pain in adolescent cross-country skiers. Cohort of 51 elite cross-country skiers from a cross-country skiing high school in Sweden participated in the study. Sagittal spinal alignment, active range of motion in flexion, extension and rotation of the thoracic spine as well as passive and active extension of the hips were measured. The participants also completed a questionnaire regarding training, competition, skiing technique and occurrence of low back pain. A simple linear regression was calculated to predict pain score based on thoraco-lumbar relation, with a significant (P<0.05) regression equation of y=-0.069x+2.280 (standard error of estimate, 0.034). Participants with greater lordosis than kyphosis were more likely to suffer from low back pain than subjects without this offset. Thoracic mobility and passive or active hip extension showed no correlation with low back pain. Sagittal spinal alignment seems to be related with low back pain among young elite cross-country skiers. This study shows that range of motion of the thoracic spine and hips do not have an effect on the prevalence of low back pain in this population.

    Adolescents are becoming less physically active and are increasingly adopting a sedentary life-style in front of computers and television screens. In this chapter we look at self-related health, physical activity, sleeping habits, the prevalence of overweight and body complaints in two senior high schools with questions about weight and height, health, physical activity, type of physical activity/sport, intensity, sleeping habits and possible injuries or complaints during the last three months. Seventy seven percent % of the high school students participated in sports on a regular basis. In academic programs compared to vocational programs, more males and females in academic programs participated in sports (71% and 80% respectively). Males reported significantly better health than females, but 65% reported body complaints during the last three months. A higher number of females than males reported back and hip complaints. Good sleep was reported in 82.1% of the males (76.6% of the females). In males 44.3% were often sleepy in the daytime (females 56.6%). Underweight, physical activity and good sleep were found to be factors with significant positive effect on good health, whereas overweight was a negative factor. Proper sleep habits and higher physical activity levels should be promoted among high school students and TV viewing time and video game use restricted. Additionally, schools should provide opportunities for young people to participate in a wider range of physical activities that address their individual needs, while promoting the health benefits of engaging in regular exercise.

METHODS: Participants were 466 high school students aged 15-17 years enrolled in academic and vocational programs. A questionnaire was completed at two senior high schools with questions about weight and height, health, physical activity, type of physical activity/sport, intensity, sleeping habits, and possible injuries or complaints during the last three months. RESULTS: Seventy seven percent of the high school students participated in sports on a regular basis. Compared with vocational programs, more males and females in academic programs participated in sports (71% and 80% respectively) (p = .036). Males reported significantly better health than females (p < .0001). 65% of the study group reported body complaints during the last 3 months. A higher number of females than males reported complaints about the back (p = .007) and the hip (p = .05). Good sleep was reported in 82.1% of males and in 76.6% of females. In males, 44.3% were often sleepy in the daytime (females 56.6%, p < .01). CONCLUSIONS: Underweight, physical activity and good sleep are factors with significant positive effect on good health, whereas overweight is a negative factor.
