    Abstract [en]

    Background and problem: Financial Fair Play was implemented in purpose to

    improve European professional football clubs’ poor financial situation by protecting the

    clubs against overinvestment's. Researchers argue theoretically that Financial Fair Play

    will affect competitive balance, but practically this is not tested.

    Purpose: The purpose of this study was to investigate whether the impact of

    competitive balance after implementation of economic regulation, depends on the

    regulation or if there are other explanations.

    Method: We have performed a quantitative study in which we tested the study´s

    purpose through different measures of competition and several statistical analyzes.

    Conclusions: We did succeed to prove that the competitive balance worsened in the

    four major European football leagues, after the implementation of Financial Fair Play.

    However, we could not substantiate that the Financial Fair Play affected the competitive

    balance. Because there are other factors that have affected. One reason could be that the

    Financial Fair Play are relatively new and may not reach its full effect.

    Implementering av patientlagstiftning: En lång resa från goda intentioner till nya förhållningssätt i vårdvardagen2017Report (Other academic)

    Den här rapporten belyser och analyserar svårigheterna att implementera lagstiftning som tillkommit under senare år för att ge patienter bättre möjligheter att påverka den vård de får. I patientmaktsutredningens delbetänkande om Patientlag (SOU 2013:2) uppmärksammas eftersläpning i implementeringen av flera av de åtgärder som trädde i kraft år 2010 som vårdgaranti, fast vårdkontakt och förnyad medicinsk bedömning (prop. 2009 /10:67 Stärkt ställning för patienten). De mest framträdande lagändringar som skett under senare år för att stärka patienters ställning är rätten till vårdval, det vill säga att välja vårdgivare inom och utom regionen (SFS 1982:763), rätten att få en förnyad medicinsk bedömning en så kallad second opinion (3 a § SFS 1982:763), rätten att få vård inom viss tid, den så kallade vårdgarantin (SFS 1982:763), samt möjligheten att välja vårdgivare inom EU. Sammantaget handlar det om åtgärder som ska stärka patientens ställning och ge den enskilde patienten inflytande över hälso- och sjukvården (SOU 2013:44).

    Frågor om patienters inflytande i vården är inte nya. De har successivt vuxit fram. Ett viktigt steg togs redan år 1990 då patienter i de flesta landsting fick möjlighet att välja vård utanför det egna landstinget. Det dåvarande Landstingsförbundet rekommenderade då landstingen att utöka patientens valmöjligheter av utförare över landstingsgränserna (SOU 2013:2). Sedan dess har policyer för att stärka patienters inflytande, engagemang och delaktighet i sin egen vård och hälsa uppmärksammats i flera utredningar och lagförslag.

    Denna rapport analyserar implementeringen av patientlagstiftning. Syftet är dels att utifrån fallstudier belysa hur lagstiftning om vårdens skyldigheter i förhållande till patienter implementeras och dels att analysera hur och varför utfallet varierar, för att kunna diskutera möjliga sätt att förstärka implementeringen av denna lagstiftning. Syftet konkretiseras i dessa övergripande forskningsfrågor:

    1. Hur påverkas implementeringen av patientlagstiftning av det regionskommunala självstyret?
    2. Hur påverkas implementeringen av patientlagstiftning av lagstiftningens utformning?
    3. Hur bidrar eller hindrar sjukvårdens professioner implementeringen av lagstiftningsåtgärder som syftar till att stärka patienters inflytande i vården?

    I denna uppsats undersöktes vilken förståelse en grupp elever i gymnasiets första år har försambandet mellan kraft och rörelse vid fritt fall. 59 elever på teknikprogrammet fick svara på tvåfysikuppgifter. I första uppgiften fick eleverna svara på vilka krafter som verkar på en boll som rörsig fritt i en kastparabel. I den andra uppgiften skulle eleverna jämföra tiden det tar för två kulor attfalla till golvet, då de rullas ut med olika hastigheter från en horisontell yta. Elevsvar jämfördes medresultat från studier som kartlagt vanligt förekommande missuppfattningar, så kalladevardagsföreställningar, bland elever som studerar fysik.Resultatet visade att den mest prevalenta uppfattningen bland eleverna var att det alltid råder enkraft i ett föremåls rörelseriktning, oavsett om farten ökar eller minskar. Denna uppfattning stämmeröverens med den så kallade impetusteorin, som introducerades före den newtonska mekaniken.Dock ansåg de flesta elever att en nedåtriktad tyngdkraft alltid råder, oavsett ett föremålsrörelseriktning. Felaktig uppfattning om krafter som orsak till rörelse ledde även till att nästanhälften av eleverna ansåg att det i uppgift 2 tar olika lång tid för kulorna att falla till golvet, om derullas ut med olika hastigheter från ett horisontellt bord. Nästan inga elever hänvisade i sinamotiveringar till acceleration i någon av uppgifterna, vilket indikerar att de inte har förståttdefinitionen av kraft. En möjlig orsak till att elever misstolkar storheten kraft är att kopplingen tillacceleration inte tydliggörs från början, då enbart statikproblem behandlas.

    Abstract [en]

    Three automatic naming tasks (Wiig & Nielsen, 1999) were administered to 60 normally functioning adults. The mean time required for naming 40 single-dimension (colors, forms, numbers, and letters) and 40 dual-dimension stimuli (color-form, color-number, and color-letter combinations) were compared in young (17-38 yr.) and older (40-68 yr.) men and women. Analysis of variance for the combined groups indicated significant naming-time differences for age but not for sex. There were no significant interaction effects. For men there was a significant naming time difference between age groups for forms, and for women for colors and forms. The sex-specific analyses indicated no significant differences in naming time based on age groups for color-form, color-number, or color-letter combinations. In a second study of adult subjects (n = 14), functional brain activity was measured with regional cerebral blood flow during the performance of the color, form, and color-form naming tasks. One subject was repeatedly measured during the performance of each task, whereas 13 subjects were measured during the performance of color-form naming. In comparison to normal reference values for rest and FAS verbal fluency, blood-flow measurements showed a consistent parietal-lobe activation during form and color-form naming, but only a slight activation during color naming. During all naming tasks, a significant frontal and frontotemporal flow decrease was seen in comparison to both rest and verbal fluency reference values. This functional brain activation pattern of a parietal increase and a frontotemporal decrease was consistently confirmed across subjects during the color-form naming task.

    Studien baseras på intervjuer av fyra nyanställda sjuksköterskor vid en vårdcentral. Syftet med studien är att belysa sjuksköterskors upplevelser av introduktion vid nyanställning, med särskilt fokus på introduktionens funktion av att bli en länk mellan personens nya arbetsuppgifter och tidigare erfarenheter, samt för att se om det finns en koppling mellan introduktionen och viljan att arbeta kvar i verksamheten. Metoden som använts i studien har utgått från en kvalitativ forskningsstrategi med fenomenologisk och hermeneutisk utgångspunkt. I resultatet kan vi se den progressiva pedagogiken inom ramen för det pedagogiska problemet, att kommunikationen är viktig samt att det finns ett behov av särskilt fokus på introduktionens funktion av att bli en länk mellan personens nya arbetsuppgifter och tidigare erfarenhet.

    Abstract [en]

    The aim of this study was to examine the experience, knowledge and attitude to mandatory reports of mistreatments amongst first line managers and assistance nurses in municipal elderly care. The following questions were at the center of the analysis: how do the different professional groups view having to report their own organization? How can various factors affect the mandatory reporting of mistreatments? What knowledge do the different professional groups have of mistreatments and mandatory reporting? A qualitative research method was used to examine these questions in the form of semi structured interviews and a small document study. The interviews were carried out with assistance nurses and first line manages in elder care, we interviewed four respondents from both professional groups. The interviews were then transcribed and coded with help of a content analysis. We have used Berger and Luckmanns theories about institutionalization when analyzing our results. We have also used Lipskys theory about street-level bureaucracy. The analysis is structured around themes and analyzed based on previous research and the above said theories. Our results show that there is a struggle amongst the both first line managers and assistance nurses to fully comprehend the complexity of mandatory reporting of mistreatments and that it’s often hard to draw the line between mandatory reporting of mistreatments and other less severe reporting’s such as deviation reports. Further, we have concluded that there is a fear amongst assistance nurses of reporting. Primarily because of a belief that reprisals, first and foremost from colleagues, will follow. We have also concluded that the workplace often seems to be characterized by a normative system in which loyalty and silence are common elements which may inhibit reporting.

    Abstract [en]

    Social work is a wide term but often presented as the form of activities done by public welfare organisations. This study aims to present an analys of social work done by the Church of Sweden congregations. The study uses a discourseanalys perspectiv to study local congregations’ instructional documents that describe their diaconia. The result shows that social work in diaconia is characterized by different hegemonies and conflicts of understanding and practices. The study shows how church of Sweden formulation can be influenced by history and political and ideological issues as well as economical issues for the church. The author means that we need more studies of how social work is understood by other welfare actors in Sweden in order to develop a deeper understanding of social work.

    Aim Our aim is to highlight perceptions of fibromyalgia among health care personell.

    MethodsA systematic literature review conducted with a deductive approach.

    FindingsHealth care personell felt insecure because of a lack of understanding which lead to avoiding contact with these patients. Many felt that the fibromyalgia patient was categorised and that they would have been better served with another name of their disease. There was a great distrust against the diagnosis and its aetiology. The patient was perceived as troublesome, illness-fixated and draining the personell of energy. The paradox that the patient is looking so healthy but bearing so much pain was confusing for the health care personell.

    Conclusions Communication and an empathic encounter was identified as important elements for patient care. The health care personell should try to look past their own perceptions of fibromyalgia and meet the patient with affirmation and respect.

    Denna studie undersöker brister i ordförståelsen i ämnet historia hos elever med svenska somandraspråk. Forskningsfrågorna är:Vilka ord tycker eleverna, både elever med svenska som andraspråk och elevermed svenska som modersmål, är svåra i historieboken?Vilka ord tror lärarna att eleverna har svårt med?Hur korrelerar lärarnas uppfattningar med elevernas?Sjutton elever med svenska som andraspråk läste två textutdrag ur två läromedel i historia A.De markerade ord som de inte förstod. För undersökningen valdes text 2 ut. För att ta reda påhur lärare tänkte om vilka ord som var svåra i ämnet historia fick dessutom fyra läraremarkera vilka ord de trodde att eleverna hade svårt att förstå. Utifrån de markerade svåraorden genomfördes ett kvalitativt ordkunskapstest med utvalda småord som eleverna ochlärarna i stort sett inte hade markerat. Denna fick gruppen av elever med svenska somandraspråk men också en kontrollgrupp av elever med svenska som modersmål besvara.Lärarna markerade olika många ord, flest ämnesspecifika, men trodde att eleverna förstod90–98 % av hela texten. Flest ord markerade läraren i svenska som andraspråk och minst ordmarkerade modersmålsläraren. Eleverna med svenska som andraspråk markerade 36 ord ocheleverna i kontrollgruppen 17. De markerade att de förstod 80–85 % av texten.Resultatet i ordkunskapstestet visar tydligt att kontrollgruppen i stor utsträckning förstårorden i testet och är cirka 20 till 35 poäng bättre än elever med svenska som andraspråk. Detär dessutom liten eller ingen skillnad i ordförståelsetestet mellan elever med svenska somandraspråk, oavsett om de varit 1,5 år eller 17 år i Sverige. Sammantaget betyder detta attelever med svenska som andraspråk förstår betydligt mindre av texten än de säger sig göra,men detta gäller också för elever med svenska som modersmål. Ytterligare ett resultat är attlärarna i denna undersökning enbart har en vag uppfattning om vilka ord som elever medsvenska som andraspråk inte förstår, vilket kan vara ett pedagogiskt problem.

    Syftet med examensarbetet var att studera och beskriva hur lärare och elever på en högstadieskola uppfattar demokrati och elevinflytande samt deras upplevelser av detsamma.

     

    I de teoretiska utgångspunkterna belyses elevinflytande i praktiken och olika dimensioner av elevinflytande. De teoretiska utgångspunkterna fokuserar också på olika aspekter av elevinflytande såsom elevinflytande för att främja lärande, för att fostra demokrater samt elevinflytande som en fråga om makt. De teoretiska utgångspunkterna avslutas med en stegmodell där inflytande graderas.

     

    I examensarbetet har jag använt mig av en kvalitativ metod som gjort det möjligt att försöka förstå och tolka respondenternas livsvärld. Empirin som har legat till grund för examensarbetet är intervjuer med sex elever i år nio samt tre undervisande lärare.

     

    Resultatet av undersökningen visar att samtliga respondenter hade en uppfattning om vad demokrati är. Lärarna tenderade att diskutera begreppet i vidare perspektiv medan eleverna uppehöll sig kring demokrati som beslutsfattande. Det praktiska elevinflytandet framträder på skolan främst som formellt inflytande - ett formellt inflytande som ofta endast har en rådgivande funktion.

    BackgroundMedical education leaders are important for educational quality in postgraduate medical education. Their work tasks are complex and contain different components. However, factors that are influencing leaders effectiveness in completing these tasks are unexplored. Understanding and developing these factors is most likely essential to strengthen postgraduate medical education and to consequently improve the quality in health care delivery. This study explores the experiences of factors that influence effectiveness of clinical consultants responsible for postgraduate medical education at clinical departments. Effectiveness was defined as fulfillment of work tasks.MethodsA qualitative study was performed with data gathered through semi-structured face-to-face interviews with 17 consultants responsible for postgraduate medical education. Data was analyzed by qualitative content analysis.ResultsFindings clustered into four themes of factors influencing effectiveness: individual (being an expert, social competence), relational (support and cooperation, communication), attitudinal (shared vision, organizational values, colleagues' attitudes) and structural (organizational characteristics, regulations and guidelines, conditions for the role). The factors were experienced to influence effectiveness in a positive or a negative direction.Conclusions This study shed light on the complex and interrelated factors experienced to have impact on the role of consultant responsible for postgraduate medical education. Viewing the result through the concept of power, the role mainly relies on personal power sources like expert and referent power whereas power connected to the position often are lacking. To increase effectiveness of the role, a differentiated strategy which involves activities at both individual, group and organizational levels is needed.

    Abstract [en]

    Our aim was to investigate gender difference in experience of equal conditions at work for women and men at a technical consultancy organization with 739 employees. Earlier studies have shown differences in work conditions for women and men (Harris, Gilbreath & Sunday, 2002; Marongiu & Ekehammar, 1999; Wicks & Bradshaw, 1999). We investigated if gender differences could be explained by four predictor variables: workload, salary and other forms of rewards, workplace climate and development opportunities. To collect data we distributed a survey to all employees (N=418). A group comparison confirmed that women experienced less equal conditions for the genders than men did. Multiple regression analyses for women showed that our model predicted 47.2% of the variance displayed in the criterion variable. All four predictor variables were significantly related with women’s experience of equal condi-tions at work. For men the model predicted 9.5% of the variance displayed in the criterion variable. Workplace climate and development opportunities were significantly related with men’s experience of equal conditions at work. The results are discussed from both an organ-izational and society view in relation to earlier findings.Keywords: workload, workplace climate, gender difference, salary and other forms of rewards, development opportunities.

    Abstract [en]

    The increasing exploitation of land and the changes in land use over the last few hundred years has led to a marked reduction in the biodiversity of the landscape. Species populations have diminished in number or even gone locally extinct due to habitat loss. Research has shown that military training areas have become a refuge for many endangered species, partly due to the fact that these areas have been spared from exploitation of the land and partly due to the fact that these species have thrived as a result of the disturbance of habitat caused by military activity.

     

    This study aims to clarify which types of disturbance have had a positive effect on the biodiversity of Torhamns udde in South East Blekinge through history. In this investigation biodiversity is analyzed at the landscape and species levels. Earlier results from a pollen analysis north of the study area show that the region has been affected by land use over a period of several thousand years. The military activity that has taken place in the area over the last hundred years, has contributed to the preservation of the unique natural environment that exists there today. An important conclusion from this study is that changes in biodiversity are strongly connected with changes in land use. The present composition in the flora and fauna may thus largely be explained by the historical land use. In order to positively influence the biodiversity of a military training area, it is important to have a palaeoecological approach, when conservation and management plans are being set up.

    Abstrakt

     

    Bakgrund: Antalet ambulansuppdrag i Sverige har ökat med 30 % under en fyra års period. I korrelation med detta har arbetsbelastningen ökat på akutmottagningarna, vilket riskerar patientsäkerheten. Studier har påvisat att en viss mängd ambulanstransporter till akutmottagningar inte är nödvändiga ur ett medicinskt perspektiv.  

     

    Syfte: Syftet med studien är att beskriva ambulanssjuksköterskors erfarenheter av att ta patienter till akutmottagning trots att bedömning gjorts att något akut medicinskt vårdbehov ej föreligger.

     

    Metod: Studien baseras på en kvalitativ metod. Intervjuer med ambulanssjuksköterskor från södra Sverige låg till grund. Intervjuerna spelades in, transkriberades och senare analyserades i enlighet med en kvalitativ innehållsanalys.

     

    Resultat: Det finns flertal faktorer som leder till att icke akut sjuka patienter transporteras till akutmottagningen. Individuella egenskaper, externa faktorer såsom riktlinjer och att läkaren påverkar sjuksköterskans beslut. Miljö som antingen är dålig för patienten men också för personalen. Hot gentemot vårdpersonalen och risken att det egna anseendet påverkas. Samt att primärvården inte har möjlighet att ta emot patienten.

     

    Diskussion: Ambulanssjuksköterskans förmåga att kommunicera med patienten kan ha en inverkan som leder till transport, även om det inte finns ett akut vårdbehov. Studier har påvisat att många larm gällande patienter utan akut medicinskt vårdbehov kan leda till att ambulanssjuksköterskan förlorar empati. Något som skulle kunna vara relevant i beslutsfattandet om vilken vårdnivå som är lämpligast för patienten.

     

    Slutsats: Ambulanssjuksköterskors arbetsmiljö påverkar beslutsfattandet. Oavsett om det är patienten, anhöriga eller andra influenser. Primärvårdens belastning tvingar dem att neka patienter. Detta leder till en ökad arbetsbelastning på akutmottagningen vilket kan riskera patientsäkerheten.

    Vårt arbete handlar om elever i sorg och hur pedagoger bemöter dem i undervisningssituationer. Arbetet är begränsat till elever i åldrarna sju till tio år. Vi har dessutom undersökt samarbetet mellan skola, kyrka och begravningsbyrå. Syftet med arbetet är att lyfta fram betydelsen av pedagogers bemötande av sorgedrabbade elever, samt att granska hur dagens verksamma pedagoger är förberedda inför en sorgesituation. Utifrån litteraturstudier har vi funnit utgångspunkten i vårt arbete. Vi har använt oss av kvalitativa intervjuer för att samla in vår empiri. Dessa har genomförts med präst, begravningsentreprenör, rektor samt tre pedagoger. Resultatet av vår undersökning visar att de intervjuade pedagogerna har en otillräcklig kunskap gällande sorgearbete. Vår studie påvisar även att samarbetet mellan skola, kyrka och begravningsbyrå varierar, från obefintligt till omfattande.

    Bakgrund: Sömn är ett grundläggande behov som påverkar vår hälsa. Bristfällig sömn kan leda till försämrat minne, koncentrationssvårigheter och dåligt humör. Inneliggande patienter med sömnbrist får ofta sämre tålamod, har svårare att hantera oro, mobiliserar sig inte lika bra och ökar risken för skada och fall. En personcentrerad vård kan främja patientens sömn då omvårdnadsåtgärderna anpassas utefter patientens individuella behov.

    Syfte: Syftet var att beskriva vuxna patienters erfarenheter av sömnstörande faktorer inom somatisk vård.

    Metod: En litteraturstudie bestående av åtta vetenskapliga artiklar med kvalitativ och kvantitativ ansats. Artiklarna kvalitetsgranskades och det utfördes en innehållsanalys av artiklarnas resultat.

    Resultat: I resultatet framkom två kategorier: Patientrelaterade faktorer och Omgivningsrelaterade faktorer. Ett flertal faktorer påverkade inneliggande patienters sömn. Inom vårdmiljön var ljud och ljus de främsta faktorerna som påverkade sömnen negativt. Dåligt patientbemötande och brist på information från sjuksköterskan skapade oro och ångest hos patienten.

    Slutsats: Ett gott bemötande och bra information skapar trygghet hos patienten. Genom att arbeta personcentrerat kan sjuksköterskan synliggöra sömnstörande faktorer och därmed främja patientens sömn.

    Syftet med studien är att bidra med kunskap om barns uppfattning av samlingen där barns perspektiv är centralt. De två forskningsfrågor som besvaras i studien är; Hur uppfattar de deltagande barnen samlingens funktion? och Vilka uppfattningar har de deltagande barnen av samling som form? Den insamlade datan analyseras genom det sociokulturella perspektivet med tillhörande teoretiska begrepp. Studien tar även inspiration från fenomenografin genom att data analyseras med fokus på att urskilja kvalitativt olika sätt som barn uppfattar, erfar eller förstår samlingen i förskolan. För att synliggöra barns uppfattningar har semistrukturerade intervjuer genomförts där elva barn medverkade på en förskola i södra Sverige. Studiens resultat visar att barns uppfattningar av samlingens funktion varierar där en del barn förknippade samlingens funktion som krav, medan andra såg möjligheter. Barns uppfattningar av samlingens form visar att samlingen är ett dagligt återkommande inslag i förskolan. Resultatet visar att det är pedagogen som har den bestämmande rollen i samlingen, men att barnen har möjlighet till inflytande i vissa situationer, om möjligheten ges av pedagogerna. Kommunikationen visar sig ha en central roll för barnens inflytande eftersom flera av barnen använder sig av sitt verbala språk och då skapar möjlighet att påverka i samlingen.

    Syftet med detta arbete är att undersöka vad musiklärare som arbetar i grundskolans tidigare år har för förhållningssätt till musikämnets egenvärde. Detta utreds genom två frågeställningar som fokuserar på syftet till att musik används i skolan samt hur musikämnets egenvärde skulle kunna öka. Forskningsmetoden som används för att undersöka detta är hermeneutisk metod och undersökningsinstrumentet är kvalitativa semistrukturerade intervjuer med fyra musiklärare. Resultatet visar att musikundervisning bedrivs i skolan i syfte att följa styrdokumenten, att generera spelglädje och kreativitet, att ge eleverna avbrott från teoretisk undervisning samt att skapa lustfyllt lärande. Musikundervisning bedrivs även i syfte att vara kultur- och traditionsbärande och har en stor betydelse för elevers identitetsskapande. Resultatet visar dessutom att studiens lärare anser att musikämnets egenvärde skulle kunna öka om dess förtjänster skulle tydliggöras, liksom de positiva resultat som musikundervisningen genererar också i andra ämnen. Egenvärdet anses även kunna öka om musikundervisningen blir mer kvalitativ genom att musiklärarna får adekvat utbildning och att de rumsliga och materiella förutsättningarna för undervisningen blir bättre. Analysen visar dock att vad lärarna benämner vara av egenvärde inte alltid är detta.

    Background

    Abuse against women causes much suffering for individuals and is a major concern for society. We aimed to estimate the prevalence of three types of abuse in patients visiting gynaecology clinics in five Nordic countries, and to assess the frequency with which gynaecologists identify abuse victims.

    Methods

    We did a cross-sectional, multicentre study of women attending five departments of gynaecology in Denmark, Finland, Iceland, Norway, and Sweden. We recruited 4729 patients; 3641 (77%) responded and were included in the study. Participants completed a postal questionnaire (norvold abuse questionnaire) confidentially. Primary outcome measures were prevalences of emotional, physical, and sexual abuse, and whether abused patients had told their gynaecologist about these experiences. We assessed differences between countries with Pearson's χ2 test.

    Findings

    The ranges across the five countries of lifetime prevalence were 38–66% for physical abuse, 19–37% for emotional abuse, and 17–33% for sexual abuse. Not all abused women reported current ill-effects from the abusive experience. Most women (92–98%) had not talked to their gynaecologist about their experiences of abuse at their latest clinic visit.

    Interpretation

    Despite prevalences of emotional, physical, and sexual abuse being high in patients visiting gynaecology clinics in the Nordic countries, most victims of abuse are not identified by their gynaecologists. This lack of discussion might increase the risk of abused patients not being treated according to their needs. Gynaecologists should always consider asking their patients about abuse.

    At present, health care staff do not seem to have sufficient knowledge about their patients' abusive experiences. The aim of the present study is to analyze and discuss what the implications might be for the encounter between patients and health care professionals, when experiences of abuse are concealed. The methodology of this article is varied: a personal narrative, medical records, sociological theoretical literature and empirical evidence. From the narrative we learn that concealment of abuse was devastating for the patient. She was “treated” in vain as a correct diagnosis was not made, while abuse by her father continued. Health care staff also violated her, which she told her therapist, but her protests were not acknowledged. Ten years of treatment thus made her even more sick. This case story focuses on the mechanisms which nurture concealment of a patient's history of abuse, such as structural and symbolic violence. We also suggest ways to break “cycles of abuse”. Help the patient to stop concealing also means that she/he leaves a victim role, gets in charge of the situation and takes a first step towards empowerment. In this way, health care settings can become enabling and empowering environments.

    Abstract [en]

    Computational enzyme design holds great promise for providing new biocatalysts for synthetic chemistry. A strategy to design small mutant libraries of complementary enantioselective epoxide hydrolase variants for the production of highly enantioenriched (S,S)-diols and (R,R)-diols is developed. Key features of this strategy (CASCO, catalytic selectivity by computational design) are the design of mutations that favor binding of the substrate in a predefined orientation, the introduction of steric hindrance to prevent unwanted substrate binding modes, and ranking of designs by high-throughput molecular dynamics simulations. Using this strategy we obtained highly stereoselective mutants of limonene epoxide hydrolase after experimental screening of only 37 variants. The results indicate that computational methods can replace a substantial amount of laboratory work when developing enantioselective enzymes.

    Syftet med vår studie var att undersöka hur lärarna arbetade med olika representationsformer inom matematikundervisningen i årskurs 1-3. Vi samlade in vårt empiriska material genom intervjuer av klasslärare samt observationer där dessa lärare undervisade i matematik. Resultatet visar att pedagogerna använder sig av olika representationsformer i sin undervisning. Det visar också att representationsformer används som ett hjälpmedel när nya begrepp ska introduceras för eleverna. Växlingarna mellan representationsformer fördjupar elevernas förståelse för matematiken och hur tal och räknesätt hänger ihop. Resultatet visar också att pedagogerna i studien ser matematikens abstrakta symboler som slutmålet i arbetet med representationsformer.

    Continuous Delivery is a software development discipline where the software is always kept in a release ready state. It has proven to be a challenge for companies to adopt the practices of Continuous Delivery, but the benefits it brings may well be worth overcoming the challenges the adoption process brings. But the problem with the challenges is that they appear to be unknown, different adoption cases report different problems, some even consider something a solution whilst another case considered the same subject to be a problem. Thus adopting Continuous Delivery is a tricky process. But the tech company IST is interested in adopting Continuous Delivery and are looking to take a soft start by adding new functionality to their service-oriented system, this functionality is to be able to run multiple versions of their services at the same time. This study has implemented this functionality in their system and then researched possible issues or benefits the functionality would have towards Continuous Delivery. Finally, a discussion was made on how the author thinks approaching Continuous Delivery should be done by any company or developer interested.

    The aim of this study was to compare the impact of contact lens cleaning on cornea using two different methods. The study compared the manufacturer’s recommended usage for Opti-Free® Express®, with the alternative cleaning method that involves rubbing the lens, a method often recommended by Optometrists.

    The participants were seen twice to evaluate the cornea. The values found on the first visit were used as baseline values. The participants were given a new pair of contact lenses and were told to clean them in two different ways for the upcoming two weeks. The first method was to follow the recommended instructions by the manufacturer. A method that involves rinsing the lens for five seconds per side. The alternative method was to rub both sides in shape of the letter L. This procedure was repeated five times per side followed by rinsing for five seconds. The participants were seen for a second visit after two weeks. Evaluations were once again made. The values found were compared with the baseline values as well with each other. 

    In those cases where the lenses had been cleaned by rubbing, the value of found staining decreased from 140179 at the baseline to 12185 after two weeks (p = 0,68). The baseline value for the eyes where the lenses were cleaned by rinsing was 125149. Those values increased to 214179 after two weeks (p = 0,03).

    A comparison between the two groups later values showed that there was a statistically significant difference between the two types of cleaning, where the eyes that were only rinsed contained a higher amount of staining (p = 0,046).

    In those eyes where lenses had been cleaned only by rinsing there were found a higher amount of staining compared to the ones cleaned by rubbing. The difference in found staining between the two cleaning methods were statistically significant.

    Ringsberg-Kristineberg is an area that contains a great cultural history dating back to the 18th century. In this area the city of Växjö is hoping to develop a “cultural and creative centre”. To achieve this they have used a planning method called cultural planning, this method uses for example a place’s culture and heritage as a guide in the search for what makes a certain place unique. The project in Ringsberg-Kristineberg made me interested in finding out which cultural heritages that were included and excluded in the future “cultural and creative centre”. I found three documents that contained the planning process of the project in Ringsberg-Kristineberg which I used as a material for a quantitative content analysis and qualitative textual analysis. The aim in this study is to examine which cultural heritage has been included and excluded in the accompanying documentation of the cultural planning-process in Ringsberg-Kristineberg. This was done by answering the following questions: (1) What does the inclusion / exclusion of cultural heritage look like in the accompanying documentation of the cultural planning method? (2) Seen from a timeline perspective; does it differ what has been included / excluded if one compares the accompanying documentation?The result shows that the first and the third of three documents show the exact same result regarding what has been included and excluded. The second document differs, it includes less heritages compared to the other two documents. Some of the included heritages are cultivation and planting, the brewery industry, the old prison and the old school town.

    Abstract [en]

    Mooring is an routine work onboard most vessels when arriving or departing a port but it is also considered a hazardous work task that have led to some very serious accidents. The purpose of this paper is to try to understand why these serious accidents continue to happen. 24 accident reports have been analysed using the HFACS-MA model. The model is developed as a framework for human and organisational causes and factors, where the causes and factors were placed in five different levels. The result shows that the largest contributing factor is the ”Preconditions”, especially considering the material such as mooring ropes that were in poor condition and therefore leading to failure and in some cases the rope parted and killed or seriously injured the crew member. The result also shows that human factors such as mistakes and lack of attention have caused serious accidents. Additionally this study examines the existing regulations and recommendations concerning mooring operations and it becomes clear that these are not thorough enough to ensure a safe working routine onboard.

    Abstract [en]

    Sixty-six of 74 adults with Insulin Dependent Diabetes Mellitus (IDDM) examined earlier (1988), were followed up two years later (1990) with the aim of studying the influence of sociodemographic factors, long-term complications and quality of life on metabolic control. It was found that both non-cohabitance and a major discrepancy between quality of life change ratings made prospectively (difference in current QOL ratings made 1990 vs 1988) and retrospectively (assessed in 1990 and covering the same period) were associated with poorer metabolic control, and also that perceived somatic health was linked with better metabolic control. A majority of the persons with a major discrepancy in their quality of life ratings, seen as suggestive of difficulties in their realistically assessing their own life situation, was also found to be non-cohabitant and/or to have incipient nephropathy, indicating these persons to constitute a risk group. In addition, well-regulated subjects were found to rate their bodily health as higher than they did their quality of life, whereas for poorly regulated persons the opposite was the case. The results were seen as pointing to the need of paying special attention clinically to the group of persons characterized here by a major discrepancy in their quality of life change ratings and also as emphasizing the fact that living with IDDM is a balancing act for the individual between satisfying bodily demands to be well-regulated and living in such a way as to perceive quality of life.

    In a previous study of a group of 74 patients with Type I diabetes, quality of life was found to be consistently enhanced a year after transition to multiple injection therapy with the insulin pen, whereas metabolic control (HbA1C) only improved moderately. The aim of the present investigation was to examine quality of life, recent life events, and metabolic control longitudinally in this original study group over a 5-year period beginning 1 year after transition to the insulin pen. Multiple analysis of variance with a repeated-measures design was used to analyze the data longitudinally and compare metabolic control in subgroups of well-controlled and poorly regulated patients during the study period. For the group as a whole, quality of life was found to change only moderately, whereas metabolic control deteriorated significantly across time following transition to the insulin pen. The two subgroups exhibited distinct differences, however, in quality of life, recent life events, and metabolic control patterns. These findings are discussed in terms of the clinical suitability of a multiple injection regimen.

    Quality of life and metabolic control were examined in 74 adults with Type 1 diabetes mellitus in 1988. Sixty-six of the persons were followed up in 1990 with the aim of studying more closely the subgroup that showed a 'major' discrepancy between 'retrospective' and 'prospective' quality of life change ratings. 'Prospective' quality of life change was defined as the difference in assessed quality of life status between 1990 and 1988. 'Retrospective' quality of life change was assessed directly by subjects in 1990 covering the same time period. Data on quality of life, well-being, sociodemographics and metabolic control were collected on both occasions. Recent life-change events and long-term complications were also recorded in 1990. Subjects with 'major' discrepancy reported greater retrospective quality of life improvement, higher quality of life, and greater well-being, as well as lesser occurrence of negative and greater occurrence of positive recent life-change events, than did those with 'minor' discrepancy, despite their exhibiting poorer metabolic control and a higher prevalence of incipient nephropathy. The results are discussed in terms of possible coping and defence strategies.

    Aims and objectivities.  The aims were to test internal consistency and interrater reliability of Mini Nutritional Assessment during implementation of Mini Nutritional Assessment in community residential homes and to test sensitivity, specificity and diagnostic predictivity of Mini Nutritional Assessment-short form vs. Mini Nutritional Assessment.

    Background.  There is a need in clinical practice to assess nutritional status in older people and to identify those who could benefit from early intervention.

    Methods.  The two-step Mini Nutritional Assessment procedure (Mini Nutritional Assessment-short form and Mini Nutritional Assessment) was used in 127 older people admitted to eight residential homes. In three of those homes (A, B and C), registered nurses simultaneously performed the assessment procedure, after receiving education and training. The intention was to offer the registered nurses a tool for independent practice use.

    Results.  Internal consistency was 0·68 (Cronbach's alpha) (n = 127). In residential home A, B and C, the registered nurses carried out Mini Nutritional Assessment in 45 residents out of 68. The agreement level between the author's and the registered nurses’ assessments was 62% (kappa 0·41). In residential home A, B and C, the agreement level was 89%, 89% and 44%, respectively. Sensitivity, specificity and diagnostic predictivity of Mini Nutritional Assessment-short form vs. Mini Nutritional Assessment were 89%, 82% and 92%, respectively.

    Conclusions.  The two-step Mini Nutritional Assessment procedure seems to be a useful tool to identify residents in need of nutritional interventions, despite the registered nurses not carrying out Mini Nutritional Assessment in all residents and the low agreement in residential home C. It indicates that to implement and use Mini Nutritional Assessment in nursing care demands the creating necessary staff resources, such as adequate staffing, sufficient education and continual supervision.

    Relevance to clinical practice.  Because of the high sensitivity of Mini Nutritional Assessment-short form and Mini Nutritional Assessment, Mini Nutritional Assessment-short form alone might be sufficient for practice use, as its simplicity might increase its usefulness.

    Acetylcholine (ACh) esterase inhibitors like galantamine and donepezil have been tested as adjunct treatment in schizophrenia. Although ACh esterase inhibition might confer some antipsychotic activity, the role of allosteric potentiation of nicotinic ACh receptors (nAChRs), which is an additional mechanism of galantamine, remains elusive. Therefore, the potential antipsychotic-like effects of galantamine and donepezil, respectively, alone, and in combination with the dopamine D2/3 receptor antagonist, raclopride, were tested in the conditioned avoidance response (CAR) test and extrapyramidal side-effect liability was assessed with the catalepsy test. Neither galantamine nor donepezil alone suppressed CAR selectively. Galantamine, but not donepezil, enhanced the raclopride-induced suppression of CAR, predicting augmentation of antipsychotic activity. In contrast to donepezil, galantamine did not increase catalepsy, alone or combined with raclopride. These data suggest that allosteric potentiation of nAChRs may mediate the antipsychotic-like effect of adjunctive galantamine and provide support for the development of α7 nAChR-selective allosteric potentiators for schizophrenia.

    Syftet med studien är att utifrån begreppet bildskapande studera hur bild används som pedagogiskt verktyg på tre olika typer av förskolor. I inledningen beskrivs att bild har fått stå tillbaka i jämförelse med de förstärkta målområdena som matematik, naturvetenskap och teknik. En kvalitativ metod används där nio pedagoger intervjuas. De valda frågeställningarna är: Hur uppfattar pedagoger att de arbetar med bild i förskolan? Hur motiverar pedagoger bild i verksamheten, det vill säga är några specifika lärmål kopplade till metoden? Hur definierar pedagoger bild, det vill säga vad (hur), med vilket material arbetar barnen med då? Hur tänker pedagoger kring sin egen roll i barns bildskapande? Är det några skillnader hur olika typer av förskolor arbetar med bild?  I resultatet framkommer det att majoriteten av pedagoger arbetar med bild i förskolan samt att de kopplar bild till det fria skapandet i förskolans verksamhet vilket innebär att barn har tillgång till material och skapar på egen hand. Pedagogerna talar om bild som en form av berättande där samtalet får en betydelse genom att de ställer öppna frågor och har en dialog med barnen. Pedagogerna uppfattar bildskapande som processer istället för den färdiga produkten. I resultatet nämner pedagoger en variation av material som kan användas i arbetet med barns bildskapande i förskolan. Slutligen kan vi konstatera att pedagoger arbetar med bild i förskolan men att utbud och tillgänglighet av material skiljer sig mellan förskolorna. Det är intressant att uppmärksamma att pedagoger reflekterar över att bild kan ingå i de förstärkta målområdena.

    Syftet med den här studien är att undersöka hur metaforer och liknelser inverkar på ömsesidig förståelse i samtal. Som teoretisk utgångspunkt har jag använt Grices (1989) teori om konversationella implikaturer samt Lakoff & Johnsons (1980) teori om konceptuella metaforer, Stålhammars (1997) teori om förståelse av metaforer och Levinsons (1983) kritik av Grice för dennes påstående om metaforer som brott mot samarbetsprincipen vid konversation. Min analysmetod är samtalsanalys, Conversation Analysis. Materialet för uppsatsen består av transkriberade utdrag ur ett radioprogram som bjuder in lyssnare att ringa in och tala om ett förutbestämt ämne med programledaren och i det här fallet även en bisittare. Resultatet visar att det finns fall där metaforer går obemärkt förbi och inte skapar problem för den ömsesidiga förståelsen i samtalet, men det finns även exempel på att bruk av metaforer och liknelser försvårar förståelsen.

    Föreliggande studie syftar till att undersöka en lärares och dennes elevers uppfattningar om hur interaktioner i klassrummet mellan läraren och eleverna i årskurs tre stöttar utvecklingen av elevernas talspråkliga färdigheter. Studiens material består av en lärarintervju och sex elevintervjuer. Materialet analyseras med hjälp av en innehållsanalys och tolkas utifrån ett sociokulturellt perspektiv på lärande, med särskilt fokus på stöttning och proximal utvecklingszon. Resultaten visar att lärarens och elevernas uppfattningar både sammanstämmer och skiljer sig åt. I kategorierna begreppsanvändning, frågor och bekräftelse finns det både likheter och skillnader mellan läraren och eleverna, dock dominerar varken likheter eller skillnader. Kategorierna som utmärker sig mest är möjlighet till turtagande och stöttning. Det framkommer markanta likheter i lärarens och elevernas uppfattningar om vilka möjligheter eleverna ges till att interagera, t.ex. att alla elever får taltid. Gällande stöttning framträder det tydliga skillnader mellan läraren och eleverna. Medan läraren uppfattar att hon endast använder frågor för att stötta utvecklingen av elevernas talspråkliga färdigheter uppfattar eleverna även att läraren stöttar dem genom att visa, ge en ”hint”, förklara och läsa högt. Slutsatsen av studien är att lärare behöver ta del av elevers uppfattningar och infallsvinklar för att kunna skapa en förståelse för och en medvetenhet om deras synsätt, vilket i sin tur kan leda till att lärare kan individualisera sin undervisning med fokus på stöttning.
