Precision based medicine means that treatment is individualized, based on environmental, lifestyle and genetic conditions. But how do we use precision when these factors are unknown? Or when genetics is based on hypervariation, such as in the production of antibodies in autoimmune diseases? Here, we try to solve those questions by taking the precision to the protein level, that is to develop tools and drugs that will target profiles of autoantibodies of individual patients. These tools and drugs are peptides that inhibit the specific interaction of the autoantibody and the self-antigen. We are exploring a novel mechanism of action to target autoimmune diseases. Our strategy is based on the binding to and neutralization of autoantibodies, and possibly the B-cells expressing those antibodies, using specific peptide binders. The concept is new and groundbreaking and we find the first applications in fundamental research, diagnosis, and therapy of rheumatoid arthritis.These peptides are based on the discovery of i) specific modified protein sequences that trigger autoimmune responses and ii) the use of novel peptide scaffolds that improve stability and binding properties. We have shown proof of principle of one prototypic binder, which is the apparent high affinity binder anti-citrullinated protein/peptide antibodies (ACPA). In this project we aim to develop this novel concept.
