SAMMANFATTNING
Patientsäkerhet är ett aktuellt ämne, mycket arbete har lagts ner på att finna metoder och rutiner för att förhindra att vårdskador inträffar. I mitt arbete på apotek har jag läst statistik om vilka fel som gjorts och hur de inte ska upprepas. Mycket av ansvaret ligger på personalen vid receptexpediering. Stöd som används är EES, elektroniskt expertstöd, som kan fånga upp interaktioner, dubbla recept och läkemedel ej lämpliga för barn och äldre. Vad som saknas är information om personalens arbetssituation, hur är det att arbeta på ett apotek, allt runtomkring just expedieringen. Ett sätt att få veta mer om detta är att använda enkäter. Sedan omregleringen av apoteksmarknaden år 2009 har enstaka sådana genomförts. Två enkätundersökningar, en före och en efter omregleringen, behandlade just skillnaden före och efter privatiseringen, en tredje enkätundersökning skickades ut av facket med svar på frivillig basis.
Patientsäkerhet inom vården har mätts med enkäter i andra länder, men dessa är varken på svenska eller anpassade efter arbetssättet på svenska apotek. De två svenska enkäterna, som användes för att ta reda på om det fanns skillnader före och efter omregleringen, skulle också kunna anpassas till idag. Det skulle dock bli svårare att jämföra patientsäkerheten och arbetssituationen internationellt.
Syftet med detta arbete var att översätta enkäten, Community Pharmacy Survey on Patient Safety Culture (SOPSTM), till svenska, samt att validera denna översättning.
Metod och Material: Den nyaste engelska versionen av enkäten och den handledning upphovsmännen publicerat användes. Översättningen gjordes med två översättare till en första version. För att validera enkäten gjordes kognitiva intervjuer med farmaceuter anställda på apotek, under sommaren 2020, framförallt digitalt pga. pandemin.
Resultatet: Intervjuerna visar att enkäten är möjlig att förstå och besvara, även om en del förslag på omformuleringar lyftes för att öka förståelsen. I intervjuerna diskuterades också hur man arbetar för att upprätthålla en god patientsäkerhet på apotek och vilka faktorer som kan påverka detta. Faktorer som togs upp är bland annat vad händer bakom disken, hur de fysiska lokalerna, utrymmena, ser ut, rutinerna och följsamheten till dessa, påverkan från kollegor, arbetsklimatet, personalbrist samt upplevelse av ansvar. De intervjuade farmaceuterna beskrev enkäten och frågorna som relevanta för patientsäkerhetsklimatet på apotek.
Slutsats: Den svenska översättningen av enkäten upplevdes av de intervjuade farmaceuterna ta upp giltiga aspekter av patientsäkerhetsklimat. Enkätens påståenden gick också att förstå även om vissa omformuleringar föreslogs. Med vissa omformuleringar kan enkäten användas för framtida undersökningar av patientsäkerhetsklimatet på svenska apotek.

ABSTRACT
Patient safety and prevention of malpractice occuring at pharmacies is important. Pharmacy staff has a large responsibility in healthcare when dispensing prescriptions. Currently there is an electronic expert support system (EES) that prevents drug interactions from occuring. This system assists the pharmacist in the detection of drug interactions such as: for example double prescriptions and detects when medicines are unsuitable for children or the elderly.
Since the deregulation of the pharmacy market in 2009, only three surveys have been conducted to assess the current work situation at community pharmacies. One was made before the deregulation of the pharmacy market in 2009 and one after this event. These were meant to show if there was a difference before and after privatization in 2009. A third survey was sent out by the union where members could answer on a voluntary basis. However these surveys could not be used in the comparison with other countries. A new survey is currently needed and it needs to be adapted for international comparison. This will make it possible to study if there are any differences in patient safety between different countries.
The aim of this study was to translate the survey, Community Pharmacy Survey on Patient Safety Culture (SOPSTM), into Swedish, and to validate the translation.
Method and Material: The latest English version of SOPSTM and the guide provided by the original authors was used. A first version of the translation was made by two translators. To validate the survey, cognitive interviews were conducted with pharmacists employed at pharmacies, during the summer of 2020, primarily digitally due to the covid 19-pandemic.
Results: The five interviews showed that the questionnaire was possible to understand by pharmacists. The interviewed pharmacists described the questions as relevant and adressing patient safety in pharmacies. However some suggestions were made to improve the understanding of the questions. During the interviews there was also a discussion about how current work methods were used to maintain a good level of patient safety in pharmacies and the different factors that could affect this. This included factors such as: what happened behind the counter, the physical premises, what the workspaces looked like, the routines and how much compliance there was to the routines, the influence of colleagues, the work climate, staff shortages and the experience of liability.
Conclusion: Although some reformulations were proposed, the translated survey questions could be understood and the Swedish translation of the questionnaire was found to address valid aspects of patient safety by the interviewed pharmacists. The survey can be used in the future for investigations of the patient safety climate in Swedish pharmacies.
